左炔诺孕酮宫内缓释系统治疗子宫内膜增生的效果观察

Effect of levonorgestrel intrauterine sustained-release system on endometrial hyperplasia

目的探讨左炔诺孕酮宫内缓释系统(LNG-IUS)治疗子宫内膜增生的效果。方法选择104例子宫内膜增生患者,根据随机数字表法分为对照组与观察组,每组52例。对照组给予黄体酮胶囊治疗,观察组给予LNG-IUS治疗。比较两组治疗效果、雌激素水平、子宫内膜厚度、血红蛋白水平、不良反应发生率。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前,两组雌二醇(E_(2))、卵泡刺激素(FSH)、促黄体生成素(LH)、黄体酮(P)水平比较,差异无统计学意义(P>0.05);治疗后,观察组E_(2)、FSH、LH、P水平均低于对照组,差异有统计学意义(P<0.05)。治疗前,两组子宫内膜厚度、血红蛋白水平比较,差异无统计学意义(P>0.05);治疗后,观察组子宫内膜厚度小于对照组,血红蛋白水平高于对照组,差异均有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论子宫内膜增生患者采用LNG-IUS治疗效果较好,可改善患者雌激素水平,降低子宫内膜厚度,提高血红蛋白水平,抑制炎症反应,安全性高,值得临床推广与应用。

Objective To investigate the effect of levonorgestrel intrauterine sustained release system(LNG-IUS)on endometrial hyperplasia.Methods A total of 104 patients with endometrial hyperplasia were selected and divided into control group and observation group according to random number table method,with 52 cases in each group.The control group was given progesterone capsule treatment,and the observation group was given LNG-IUS treatment.The therapeutic effect,estrogen level,endometrial thickness,hemoglobin level and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of estradiol(E_(2)),follicle-stimulating hormone(FSH),luteinizing hormone(LH)and progesterone(P)between the two groups(P>0.05).After treatment,the levels of E_(2),FSH,LH and P in the observation group were lower than those in the control group,and the difference was statistically signifi cant(P<0.05).Before treatment,there were no significant differences in endometrial thickness and hemoglobin level between the two groups(P>0.05).After treatment,the endometrial thickness of the observation group was lower than that of the control group,and the hemoglobin level of the control group was higher,with statistical signifi cance(P<0.05).The incidence of adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion LNG-IUS treatment is effective in patients with endometrial hyperplasia,which can improve estrogen level,reduce endometrial thickness,increase hemoglobin level and inhibit inflammation.It is safe and worthy of clinical promotion and application.
