规范化流程护理对晚期肿瘤患者癌痛及心理应激反应的影响

Effects of standardized nursing process on the cancer pain and psychological stress reaction of patients with advanced tumors

目的分析肿瘤晚期患者的临床现状,探讨患者治疗过程中应用规范化流程护理对癌痛以及心理应激反应的影响。方法选择2019年1月至2020年1月接受治疗的80例肿瘤晚期患者,采用数字双盲法分为对照组和研究组,每组40例。对照组接受常规干预,常规干预基础上开展规范化流程护理。比较2组患者癌痛控制情况、心理应激反应、生存质量、癌因性疲乏评分。结果研究组在护理前患者SAS、SDS、QLQ-30评分差异无统计学意义(P>0.05),研究组护理后1周、2周SAS、SDS评分均低于对照组(P<0.05)。QLQ-30评分高于对照组(P<0.05),入院时患者VAS评分差异无统计学意义(P>0.05),疼痛爆发时VAS评分研究组低于对照组(P<0.05),疼痛持续时间短于对照组(P<0.05)。干预前2组癌因性疲乏评分差异无统计学意义(P>0.05),干预后研究组评分低于对照组(P<0.05)。结论肿瘤晚期患者治疗期间接受规范化流程护理干预,可有效减轻心理应激反应,缩短疼痛持续时间,缓解癌痛以及癌痛疲乏情况,提高生存质量,该模式具有较高临床应用价值。

Objective To investigate the effects of of standardized nursing process on the cancer pain and psychological stress reaction of patients with advanced tumors.Methods Eighty patients with advanced tumors who were treated in our hospital from January 2019 to January 2020 were enrolled in the study,who were divided into control group and research group according to digital double-blind method,with 40 cases in each group.The patients in control group were treated by routine nursing intervention,however,those in research group,on the basis of control group,were treated by standardized nursing process intervention.The cancer pain control status,psychological stress response,quality of life,and cancer-related fatigue score were observed and compared between the two groups.Results Before nursing intervention,there were no significant differences in the scores of SAS,SDS,and QLQ-30 between the two groups(P>0.05).At 1w,2w after nursing intervention,the scores of SAS and SDS in research group were significantly lower than those in control group(P<0.05),but,the scores of QLQ-30 were significantly higher than those in control group(P<0.05).On admission there were no significant differences in the VAS scores between the two groups(P>0.05),however,the VAS scores when the pain broke out in research group were significantly lower than those in control group(P<0.05),and the pain duration was significantly shorter than that in control group(P<0.05).Before the intervention,there were no significant differences in the scores of cancer-related fatigue between the two groups(P>0.05).After the intervention,the scores in research group were significantly lower than those in control group(P<0.05).Conclusion The standardized process nursing intervention for the patients with advanced tumors can effectively relieve psychological stress reaction,shorten the duration of pain,relieve cancer pain and fatigue,and improve the life quality of patients,therefor,the intervention mode has better clinical application value.
