宫腔镜手术治疗子宫内膜息肉不孕症的临床观察

Clinical observation of hysteroscopic surgery for endometrial polyp infertility

目的探讨宫腔镜手术治疗子宫内膜息肉不孕症的临床效果。方法选取于本院接受治疗的92例子宫内膜息肉不孕症患者为研究对象,采用随机数字表法将其分为刮宫组、电切组。刮宫组47例采用宫腔镜下刮宫术治疗,电切组45例采用宫腔镜下电切术治疗。比较两组的临床疗效及妊娠结局。结果电切组术中失血量少于刮宫组,手术时间、住院时间均短于刮宫组,差异有统计学意义(P<0.05)。术前,两组月经量比较,差异无统计学意义(P>0.05);术后1、3个月,两组月经量均少于术前,且电切组月经量均少于刮宫组,差异有统计学意义(P<0.05)。电切组临床妊娠率为86.67%,明显高于刮宫组的36.17%,未孕率为8.89%,明显低于刮宫组的46.81%,差异有统计学意义(P<0.05);两组生化妊娠率比较,差异无统计学意义(P>0.05)。结论与宫腔镜下刮宫术比较,采用宫腔镜下电切术治疗子宫内膜息肉不孕症患者的临床优势更大,可帮助患者尽快恢复卵巢功能,提高受孕成功率。

Objective To investigate the clinical effect of hysteroscopic surgery on endometrial polyp infertility.Methods 92 cases of infertility patients with intrauterine polyps who received treatment in our hospital were selected as research objects,and they were divided into a curettage group and an electrotomy group by random number table method.47 cases in the curettage group were treated by hysteroscopic curettage,and 45 cases in the electrohysterectomy group were treated by hysteroscopic electrohysterectomy.The clinical efficacy and pregnancy outcomes of the two groups were compared.Results The blood loss in the electrotomy group was less than that in the curettage group,and the operation time and hospital stay were shorter than those in the curettage group,the differences were statistically significant(P<0.05).Before surgery,there was no significant difference in menstrual volume between the two groups(P>0.05);1 and 3 months after surgery,the amount of menstruation in both groups was lower than before surgery,and the amount of menstruation in electrotomy group were lower than those in curettage group,the differences were statistically significant(P<0.05).The clinical pregnancy rate of the electrotomy group was 86.67%,which was significantly higher than 36.17%of the curettage group,and the non-pregnancy rate was 8.89%,which was significantly lower than 46.81%of the curettage group,the differences were statistically significant(P<0.05);there was no significant difference in biochemical pregnancy rate between the two groups(P>0.05).Conclusion Compared with hysteroscopic curettage,hysteroscopic electrotomy has greater clinical advantages in the treatment of infertility patients with endometrial polyps,which can help patients recover ovarian function as soon as possible and improve the success rate of conception.
