宫腔镜下子宫内膜切除联合孕激素治疗子宫内膜息肉的临床效果

Clinical effect of hystcroscopic endometrial resection combined with progesterone in the treatment of endometrial polyps

目的探究子宫内膜息肉采用宫腔镜下子宫内膜切除联合孕激素治疗的临床效果。方法选取于本院进行治疗的子宫内膜息肉患者136例为研究对象,按治疗方法不同将其分为研究组和对照组,各68例。对照组采用宫腔镜子宫内膜息肉切除术治疗,研究组在术后采用地屈孕酮片治疗。比较两组患者术前及术后3、6、12个月的月经量、子宫内膜厚度及复发率。结果术前,两组月经量比较,差异无统计学意义(P>0.05);手术后3、6、12个月,研究组月经量均少于对照组,差异有统计学意义(P<0.05)。术前,两组子宫内膜厚度比较,差异无统计学意义(P>0.05);手术后3、6、12个月,研究组子宫内膜厚度均小于对照组,差异有统计学意义(P<0.05)。研究组总复发率为4.41%,明显低于对照组的14.71%,差异有统计学意义(P<0.05)。结论地屈孕酮片联合宫腔镜下子宫内膜息肉切除术有助于提高子宫内膜息肉的治疗效果,促进患者康复,降低术后复发率,值得临床推广使用。

Objective To investigate the clinical effect of hysteroscopic endometrial resection combined with progesterone in the treatment of endometrial polyps.Methods 136 patients with endometrial polyps treated in our hospital were selected as the study objects,with 68 patients in each case.They were divided into study group and control group according to different treatment methods.The control group was treated with hysteroscopic endometrial polypectomy,and the study group was treated with dydrogcstcrone tablets after surgery.The menstrual volume,endometrial thickness and recurrence rate before and 3,6 and 12 months after operation were compared between the two groups.Results Before operation,there was no significant difference in menstrual volume between the two groups(P>0.05);at 3,6 and 12 months after operation,the menstrual volume of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).Before operation,there was no significant difference in endometrial thickness between the two groups(P>0.05);at 3,6 and 12 months after operation,the endometrial thickness of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).The total recurrence rate of the study group was 4.41%,which was significantly lower than 14.71%of the control group,the difference was statistically significant(P<0.05).Conclusion Dydrogesterone tablets combined with hysteroscopic resection of endometrial polyps can improve the therapeutic effect of endometrial polyps,promote the recovery of patients,reduce the postoperative recurrence rate,and is worthy of clinical promotion.
