来曲唑与克罗米芬治疗多囊卵巢综合征不孕症临床观察

Clinical observation of lctrozolc and clomiphcnc in the treatment of polycystic ovary syndrome infertility

目的分析来曲唑及克罗米芬治疗多囊卵巢综合征(PCOS)不孕的疗效。方法选取本院收治的PCOS不孕症患者68例,以抽签法分为对照组及研究组,每组34例。对照组采用来曲唑治疗,研究组采用克罗米芬治疗,比较两组的性激素水平、子宫内膜厚度、排卵和妊娠情况。结果治疗前,两组促黄体生成素(LH)、促卵泡生成激素(FSH)、雌二醇(E_(2))及睾酮(T)水平比较,差异无统计学意义(P>0.05);治疗后,研究组E_(2)水平明显低于对照组,差异有统计学意义(P<0.05),两组LH、FSH、T水平比较,差异无统计学意义(P>0.05)。研究组子宫内膜厚度值为(9.58±2.14)mm,高于对照组的(7.26±1.85)mm,差异有统计学意义(P<0.05)。研究组排卵率低于对照组,生化妊娠率、临床妊娠率均高于对照组,差异有统计学意义(P<0.05);两组成熟卵泡个数比较,差异无统计学意义(P>0.05)。结论在PCOS不孕症的治疗中,克罗米芬的促排卵效果优于来曲唑,但来曲唑能提高子宫内膜厚度及妊娠率,总体效果优于克罗米芬。

Objective To analyze the efficacy of letrozole and clomiphene in the treatment of polycystic ovary syndrome(PCOS)infertility.Methods 68 cases of PCOS infertility patients admitted to our hospital were selected and were divided into a control group and a study group by lottery method,with 34 cases in each group.The control group was treated with letrozole and the study group was treated with clomiphene.The sex hormone levels,endometrial thickness,ovulation and pregnancy were compared between the two groups.Results Before treatment,there were no significant differences in the levels of luteinizing hormone(LH),follicle-stimulating hormone(FSH),estradiol(E_(2))and testosterone(T)between the two groups(P>0.05);after treatment,the E_(2) level of the research group was significantly lower than that of the control group,the difference was statistically significant(P<0.05),but there were no statistical significances in LH,FSH and T levels between the two groups(P>0.05).The endometrial thickness of the study group was(9.58±2.14)mm,which was higher than(7.26±1.85)mm of the control group,the difference was statistically significant(P<0.05).The ovulation rate of the study group was lower than that of the control group,and the biochemical pregnancy rate and clinical pregnancy rate were higher than that of the control group,the differences were statistically significant(P<0.05);there was no significant difference in the number of mature follicles between the two groups(P>0.05).Conclusion In the treatment of PCOS infertility,clomiphene has better ovulation induction effect than letrozole,but letrozole can improve endometrial thickness and pregnancy rate,and the overall effect is better than clomiphene.
