中西医结合治疗急性心肌梗死合并应激性高血糖的临床效果研究

Clinical study of acute myocardial infarction complicated with stress hyperglycemia treated by integrated traditional Chinese and Western medicine

目的讨论急性心肌梗死合并应激性高血糖采取中西医结合治疗的临床疗效。方法42例急性心肌梗死合并应激性高血糖患者,根据入院顺序分为研究组与对照组,各21例。对照组接受常规胰岛素治疗,研究组在对照组治疗基础上加用化浊解毒汤进行治疗。比较两组治疗后的相关指标、低血糖发生率。结果治疗后,研究组的N末端脑钠肽前体(618.65±31.66)ng/L、超敏C反应蛋白(21.27±1.48)mg/L、空腹血糖(6.00±0.37)mmol/L低于对照组的(711.54±31.87)ng/L、(24.18±1.58)mg/L、(6.57±0.36)mmol/L,胰岛素敏感指数(-5.22±1.55)、空腹C肽(3.26±0.78)ng/ml高于对照组的(-6.24±1.56)、(2.36±0.63)ng/ml,差异均有统计学意义(P<0.05)。研究组低血糖发生率4.76%低于对照组的28.57%,差异有统计学意义(P<0.05)。结论临床中应用中西医结合治疗急性心肌梗死合并应激性高血糖疗效确切,值得大力推广与应用。

Objective To discuss the clinical efficacy of acute myocardial infarction complicated with stress hyperglycemia treated by integrated traditional Chinese and Western medicine.Methods A total of 42 patients with acute myocardial infarction and stress hyperglycemia were divided into the research group and the control group according to the order of admission,with 21 patients in each group.The control group received routine insulin treatment,while the research group was treated with Huazhuo Jiedu Decoction on the basis of the control group.The related indexes and the incidence of hypoglycemia after treatment were compared between the two groups.Results After treatment,the research group had N-terminal pro-B type natriuretic peptide of(618.65±31.66)ng/L,high-sensitivity C-reactive protein of(21.27±1.48)mg/L,and fasting blood glucose of(6.00±0.37)mmol/L,which were lower than(711.54±31.87)ng/L,(24.18±1.58)mg/L,and(6.57±0.36)mmol/L in the control group;the research group had insulin sensitivity index of(-5.22±1.55),fasting C-peptide(3.26±0.78)ng/ml,which were higher than(-6.24±1.56)and(2.36±0.63)ng/ml in the control group;the differences were all statistically significant(P<0.05).The incidence of hypoglycemia in the research group was 4.76%,which was lower than 28.57%in the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical application of integrated traditional Chinese and Western medicine is effective in the treatment of acute myocardial infarction combined with stress hyperglycemia,which should be vigorously popularized and applied.
