不同时机经皮椎体后凸成形术治疗老年骨质疏松性椎体压缩性骨折的疗效分析

Efficacy Analysis of Percutaneous Kyphoplasty at Different Timing in Treatment of Osteoporotic Vertebral Compression Fracture in Elderly

目的:分析不同时机经皮椎体后凸成形术(PKP)治疗老年骨质疏松性椎体压缩性骨折(OVCF)的效果。方法:选取2020年6月-2021年4月揭阳市揭东区第二人民医院收治的100例老年OVCF患者作为研究对象,依据随机数字表法分为对照组和观察组,各50例。对照组于骨折后7~14 d行PKP,观察组于骨折后7 d内行PKP,术后随访1个月。比较两组椎体前缘高度、疼痛程度、腰椎功能恢复情况及并发症发生率。结果:术前,两组椎体前缘高度比较,差异无统计学意义(P>0.05);术后1个月,两组椎体前缘高度均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。术前,两组疼痛、腰椎功能评分比较,差异无统计学意义(P>0.05);术后1个月,观察组疼痛、腰椎功能评分均低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:骨折后7 d内行PKP,对老年OVCF患者的治疗效果更为确切,可有效改善其椎体前缘高度,缓解疼痛,恢复腰椎功能,降低并发症发生风险,临床应用价值较高,可大力推广。

Objective:To compare the effects of percutaneous kyphoplasty(PKP)at different timing in elderly patients with osteoporotic vertebral compression fracture(OVCF).Methods:A total of 100 patients with OVCF admitted to the Second People's Hospital of Jiedong District in Jieyang City from June 2020 to April 2021 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group received PKP within 7-14 days after fracture,and the observation group received PKP within 7 days after fracture,all patients were followed up for 1 month after surgery.The anterior vertebral height,pain degree,recovery of lumbar function and incidence of complications were compared between the two groups.Results:Before surgery,there were no significant differences in anterior vertebral height between the two groups(P>0.05).At 1 month after surgery,the anterior vertebral height in two groups were higher than that before surgery,the observation group was higher than the control group,and the differences were statistically significant(P<0.05).Before surgery,there was no significant difference in pain and lumbar function scores between the two groups(P>0.05).At 1 month after surgery,the pain and lumbar function scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:PKP treatment within 7 days after fracture has more definite treatment effect in elderly patients with OVCF,it can effectively improve the anterior vertebral height,relieve pain,restore lumbar function,reduce the incidence of complications,with high clinical application value,it can be promoted greatly.
