中药房饮片调配的常见差错分析及中药处方点评干预效果

Analysis of Common Mistakes in the Blending of Chinese Herbal Medicines and the Intervention Effect of Chinese Medicine Prescription Comments

目的研究中药房饮片调配常见问题分析以及处方点评的干预效果。方法沈阳浑河新城中西医结合医院在2019年3月开始实施中药处方点评,选择干预前后一年的368张中药房饮片调配处方进行分析,对比饮片调配差错发生情况以及患者满意度评分。结果干预前共有42张处方出现差错,差错率为11.41%,干预后共有18张处方出现差错,差错率为4.89%,干预后明显比干预前低(P<0.05);干预后评分明显比干预前高(P<0.05)。结论脚注不到位、没有明确的组方结构、没有规范开具处方以及药物配伍禁忌等均是常见导致差错发生的原因,而审方、用法用量以及抓配等为主要差错类型,在明确差错类型和发生原因基础上使用中药处方点评方式可以降低差错的发生,值得推广和使用。

Objective To study the analysis of common problems in the preparation of decoction pieces in traditional Chinese pharmacies and the intervention effects of prescription reviews.Methods Hunhe Xincheng Hospital of Integrated Traditional Chinese and Western Medicine began to implement traditional Chinese medicine prescription reviews in March 2019,and selected 368 traditional Chinese pharmacy prescriptions for prescriptions before and after the intervention for analysis,and compared the occurrence of prescription mistakes and patient satisfaction scores.Results A total of 42 prescriptions had errors before intervention,and the error rate was 11.41%.After intervention,18 prescriptions had errors,and the error rate was 4.89%,which was significantly lower after intervention than before intervention(P<0.05).The score after intervention was significantly higher than that before intervention(P<0.05).Conclusion Inadequate footnotes,no clear prescription structure,no standardized prescriptions,and contraindications to drug compatibility are all common causes of errors.Examination,usage and dosage,and grasping are the main types of errors.The use of traditional Chinese medicine prescription reviews based on the cause of occurrence can reduce the occurrence of errors and is worthy of promotion and use.
