完带汤加减对HPV感染合并低度宫颈上皮内瘤变患者疾病转归及局部炎症的影响

Influence of modified Wandai decoction for disease outcome and local inflammation of patients with HPV infection complicated with low-grade cervical intraepithelial neoplasia

目的:探究完带汤加减对人乳头瘤病毒(HPV)感染合并低度宫颈上皮内瘤变患者疾病转归及局部炎症的影响。方法:将80例HPV感染合并低度宫颈上皮内瘤变患者根据随机数字表法分为两组,每组40例。对照组进行HPV感染合并低度宫颈上皮内瘤变的常规西医治疗,观察组则在对照组治疗的基础上采用完带汤加减治疗。比较两组的治疗总有效率、HPV转阴率、治疗前后的中医证候积分、局部炎症相关指标阴道灌洗液防御素[β-防御素-2(HBD-2)及防御素-5(HD-5)]及阴道灌洗液炎症因子[肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)及白介素-6(IL-6)]。结果:观察组的治疗总有效率及治疗后3个月、6个月的HPV转阴率分别为77.50%,55.00%及75.00%,高于对照组的37.50%,30.00%及47.50%(P<0.05);治疗后两组的上述中医证候积分、阴道灌洗液防御素、阴道灌洗液炎症因子均低于治疗前,且观察组的积分及阴道灌洗液指标显著低于对照组(P<0.05)。结论:完带汤加减在HPV感染合并低度宫颈上皮内瘤变患者中的应用效果较好,可有效改善患者的疾病转归及控制局部炎症。

Objective:To investigate the influence of modified Wandai decoction for disease outcome and local inflammation of patients with HPV infection complicated with low-grade cervical intraepithelial neoplasia.Methods:80 patients with HPV infection complicated with low-grade cervical intraepithelial neoplasia were divided into control group and observation group with 40 cases in each group according to the method of random number table.The control group were treated with routine western medicine treatment,the observation group were treated with modified Wandai decoction on the treatment of control group.Then the total effective rates,HPV negative rates,TCM syndrome integral and local inflammation related indexes vaginal lavage liquid defensins[β-Defensin-2(HBD-2)and Defensin-5(HD-5)]and inflammatory factors[tumor necrosis factor-α(TNF-α),Interleukin-1β(IL-1β)and interleukin-6(IL-6)]before and after the treatment of two groups were compared.Results:The total effective rates and HPV negative rates at third and sixth month after the treatment of observation group were respectively 77.50%,55.00%and 75.00%,and they were higher than 37.50%,30.00%and 47.50%of control group(P<0.05);the TCM syndrome integral,vaginal lavage liquid defensinsand inflammatory factors of two groups after the treatment were higher than those before the treatment,and the integral and vaginal lavage liquid indexes of observation group were significantly lower than those in control group(P<0.05).Conclusion:The application effect of modified Wandai decoction in the treatment of patients with HPV infection complicated with low-grade cervical intraepithelial neoplasia is better,and it can effectively improve the disease outcome and control the local inflammation of patients.
