舒肝解郁胶囊联合盐酸帕罗西汀治疗老年抑郁症临床观察

Clinical Observation on Shugan Jieyu Capsules Combined with Paroxetine Hydrochloride in the Treatment of Geriatric Depression

目的研究舒肝解郁胶囊联合盐酸帕罗西汀治疗老年抑郁症的效果、安全性及对患者肝功能的影响。方法选择2019年6月—2020年5月于大连医科大学附属第一医院接受治疗的老年抑郁症患者128例,随机分为2组,每组64例,对照组采用盐酸帕罗西汀,治疗组则在对照组基础上联用舒肝解郁胶囊,2组疗程均为12周。对比2组临床疗效、治疗前后汉密尔顿抑郁量表24项版(HAMD-24)、生活质量调查简表(SF-36)评分、肝功能指标[丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、总胆红素(TBIL)]及不良反应。结果治疗后,治疗组总有效率为93.75%(60/64),明显高于对照组的81.25%(52/64)(P<0.05);治疗组HAMD-24评分降低较对照组更显著(P<0.05);治疗组SF-36评分优于对照组(P<0.05);2组患者ALT、AST、TBIL均较治疗前无变化(P>0.05);治疗组不良反应发生率为10.94%(7/64),低于对照组的28.13%(18/64)(P<0.05)。结论舒肝解郁胶囊联合盐酸帕罗西汀治疗老年抑郁症比单一应用盐酸帕罗西汀更有效,且不引起肝损伤、安全性高。

Objective To research the effect and safety of Shugan Jieyu capsule combined with paroxetine hydrochloride in the treatment of elderly depression and its influence on liver function.Methods A total of 128 elderly patients with depression received treatment in the First Affiliated Hospital of Dalian Medical University from June 2019 to May 2020 were randomly divided into 2 groups,with 64 cases in each group.The control group received paroxetine hydrochloride and the treatment group received Shugan Jieyu capsule combined with the control group.The course of treatment in both groups was 12 weeks.The clinical effect,24-item Hamilton Depression Scale(HAMD-24),SF-36 score of Quality of Life survey before and after treatment,liver function injury[alanine aminotransferase(ALT),aspartate transferase(AST),total bilirubin(TBIL)]and adverse reactions of the 2 groups were compared.Results After treatment,the total effective rate of the treatment group was 93.75%(60/64),which was significantly higher than that of the control group(81.25%,52/64)(P<0.05).HAMD-24 score in treatment group was significantly lower than that in control group(P<0.05).The SF-36 score of the treatment group was better than that of the control group(P<0.05).ALT,AST and TBIL in 2 groups were not changed compared with before treatment(P>0.05).The incidence of adverse reactions in treatment group was 10.94%(7/64),which was lower than that in control group(28.13%,18/64)(P<0.05).Conclusion Shugan Jieyu capsule combined with paroxetine hydrochloride is more effective in treating senile depression than paroxetine hydrochloride alone.It does not cause liver injury and has high safety.
