摘要
目的探究肿瘤标志物联合细胞因子对高危人群及肺结节肺癌早期诊断的价值。方法回顾性分析本院2019年12月—2021年12月收治的100例肺癌高危患者、70例肺部良性疾病患者及75例健康体检者的临床资料,检查所有患者血清肿瘤标志物与细胞因子,通过受试者工作特征曲线(ROC)分析诊断高价值;依据肺结节良恶性分为肺结节良性组(43例)与肺结节恶性组(57例),比较2组各项水平差异,分析其诊断效能。结果肺癌高危组CEA、NSE、CYFRA21-1、IL-6、IL-8、TNF-α水平高于良性组、对照组(P<0.05);肺结节良性患者CEA、NSE、CYFRA21-1、IL-6、IL-8水平低于肺结节恶性组(P<0.05)。CEA+NSE+CYFRA21-1、IL-6+IL-8+TNF-α诊断肺癌高危患者的敏感度较高,分别为93.0%、90.0%,特异度分别为91.4%、90.0%;CEA+NSE+CYFRA21-1、IL-6+IL-8+TNF-α诊断肺癌良恶性结节的敏感度较高,分别为95.3%、93.0%,特异度分别为70.2%、77.2%。结论高危人群肺癌与肺结节恶性患者的肿瘤标志物及炎性因子水平较高,联合诊断能够有效提高诊断价值。
Objective To explore the value of tumor markers combined with cytokines in the early diagnosis of high-risk groups and lung nodular lung cancer.Methods The clinical data of 100 high-risk lung cancer patients,70 patients with benign lung diseases,and 75 healthy individuals admitted to our hospital from December 2019 to December 2021 were retrospectively analyzed.Serum tumor markers and cytokines in all patients were checked,and high value was diagnosed through receiver operating characteristic(ROC)analysis.According to the benign and malignant pulmonary nodules,they were divided into benign pulmonary nodule group(43 cases)and malignant pulmonary nodule group(57 cases)to compare the difference of each level between the two groups and analyze the diagnostic efficiency.Results The levels of CEA,NSE,CYFRA21-1,IL-6,IL-8 and TNF-αin the high-risk lung cancer group were higher than those in the benign group and the control group(P<0.05).The levels of CEA,NSE,CYFRA21-1,IL-6 and IL-8 in benign pulmonary nodules were lower than those in malignant pulmonary nodules(P<0.05).The sensitivities of CEA+NSE+CYFRA21-1 and IL-6+IL-8+TNF-αin diagnosing high-risk patients with lung cancer were high,which were 93.0%and 90.0%,and the specificities were 91.4%and 90.0%.The sensitivities of CEA+NSE+CYFRA21-1,IL-6+IL-8+TNF-αin the diagnosis of benign and malignant lung cancer nodules were 95.3%and 93.0%,and the specificities were 70.2%and 77.2%.Conclusion The level of tumor markers and inflammatory factors in high-risk groups of lung cancer and malignant pulmonary nodules is high,and the combined diagnosis can effectively improve the diagnostic value.
