乳岩宁方对绝经后乳腺癌内分泌治疗患者性激素水平、中医证候及生活质量的影响

Effect of Ruyaning Recipe(乳岩宁方) on Sex Hormone,TCM Symptoms and Quality of Life in Postmenopausal Breast Cancer Patients with Endocrine Therapy

目的 探讨乳岩宁方对绝经后乳腺癌内分泌治疗患者性激素水平、中医证候及生活质量的影响。方法 选取诊治绝经后乳腺癌内分泌治疗患者80例,随机分为对照组与观察组,每组40例,对照组给予来曲唑,观察组在对照组用药基础上给予乳岩宁方,疗程2个月,观察两组患者治疗前后血雌二醇(E_2)、促黄体生成激素(LH)、促卵泡生成激素(FSH)变化,中医证候积分变化,采用癌症治疗功能评价系统(FACT)中特异模块乳腺癌(FACT-B)量表评估患者生活质量,采用改良Kupperman症状评分对患者类更年期综合征症状进行评价。结果 两组患者治疗前和治疗后血清E_2、LH、FSH水平组内及组间比较,差异无统计学意义(P>0.05);两组治疗前中医证候积分比较,差异无统计学意义(P>0.05),观察组治疗后情绪抑郁、潮热盗汗、烦躁易怒、乏力倦怠、五心烦热、头晕耳鸣、关节疼痛、胸胁胀闷、阴道干涩等症状积分较治疗前明显降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组治疗前FACT-B量表中各项目评分比较,差异无统计学意义(P>0.05),观察组治疗后躯体状况、社会/家庭状况、功能状况、情感状况、附加关注评分较治疗前明显升高,且观察组高于对照组,差异有统计学意义(P<0.05);两组治疗前改良Kupperman症状评分差异无统计学意义(P>0.05),观察组治疗后改良Kupperman症状评分低于对照组,且低于对照组,差异有统计学意义(P<0.05)。结论 绝经后乳腺癌内分泌治疗患者采用乳岩宁方辅助治疗有助于症状及生活质量的改善,对性激素水平无明显影响。

Objective To investigate the effect of Ruyaning Recipe(乳岩宁方)on sex hormone,TCM symptoms and quality of life in postmenopausal breast cancer patients with endocrine therapy.Methods Eighty postmenopausal breast cancer patients with endocrine therapy were randomly divided into the control group and the observation group,40 cases in each group.The control group received letrozole orally.The observation group was additionally given Ruyaning Recipe orally.The treatment course was two months.The levels of blood estradiol(E2),luteinizing hormone(LH)and follicle-stimulating hormone(FSH)before and after treatment and TCM syndrome scores were observed.The quality of life was assessed by using the FACT-B specific module breast cancer scale in the Evaluation System of Cancer Treatment Function(FACT).The modified Kupperman symptom score was used to evaluate the symptoms of menopausal syndrome.Results There were no significant differences in levels of E2,LH or FSH in serum between the two groups before and after treatment(P>0.05).There was no significant difference in TCM syndrome scores between the two groups before treatment(P>0.05).The scores of symptoms such as depression,hot flashes,night sweats,irritability,fatigue and tiredness,heat in palms and soles,dizziness and tinnitus,joint pain,chest and flank distention and tightness and vaginal dryness were decreased significantly after treatment in the observation group(P<0.05)and the scores of the observation group were lower than those of the control group(P<0.05).There was no statistical significance in the scores of FACT-B scale between the two groups before treatment(P>0.05).The scores of physical status,social/family status,functional status,emotional status and additional attention were higher after treatment than those before treatment in the observation group(P<0.05),and the scores of the observation group were higher than those of the control group(P<0.05).There was no significant difference in modified Kupperman symptom scores between the two groups before treatment(P>0.05).The modified Kupperman symptom score after treatment in the observation group was lower than that in the control group(P<0.05),and the score of the observation group lower than that of the control group(P<0.05).Conclusion The endocrine therapy for postmenopausal breast cancer patients using Ruyaning Recipe can helpful to improve symptoms and quality of life,and there is no significant effect on sex hormone levels.
