项痹通颗粒对颈后肌炎症损伤大鼠细胞形态学及TGF-β、TNF-α、IL-1β、PGE_(2)、NGF的影响

摘要 目的 观察项痹通颗粒对颈后肌炎症损伤大鼠肌细胞形态学及转化生长因子-β(transforming growth factor beta, TGF-β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-1β(interleukin-1,IL-1β)、前列腺素E_2(prostaglandin E_2,PGE_(2))、神经生长因子(nerve growth factor, NGF)的影响,探讨其可能的作用机制。方法 将40只SPF级雄性SD大鼠随机分为假手术组、模型组、西药组、中药组(每组10只),以手术方式切开术口、用弯头止血钳钳夹C_(2-6)棘突旁开0.2 cm的颈后肌中段(持续钳夹10 s、后松开间隔10 s、扣满3扣)以建立颈后肌炎症损伤大鼠模型。西药组、中药组分别予以塞来昔布、项痹通颗粒灌胃,假手术组及模型组予以生理盐水灌胃。苏木素-伊红(hematoxylin-eosin, HE)染色观察大鼠颈后肌细胞的形态学变化,酶联免疫吸附法(enzyme-linked immunosorbent assay, ELISA)检测大鼠造模后第2天(干预前)及干预7 d后的TGF-β、TNF-α、IL-1β、PGE_(2)、NGF水平。结果 造模后第2天(干预前),与假手术组比较,模型组颈后肌中段颜色暗红,TGF-β、TNF-α、IL-1β、PGE_(2)及NGF显著升高(P<0.05)。干预7 d后,与假手术组比较,模型组肌细胞核减少、结构紊乱、变形水肿、大小不一,TGF-β、TNF-α、IL-1β、PGE_(2)及NGF明显升高(P<0.05);与模型组比较,西药组及中药组肌细胞核稍增多、结构变清晰、水肿程度减轻、形状较规则,以中药组改善较明显,TNF-α、IL-1β、PGE_(2)及NGF明显降低(P<0.05),而TGF-β差异无统计学意义(P>0.05),但数据有所下降。结论 项痹通颗粒可改善颈后肌炎症损伤大鼠肌细胞的病理学变化、减轻炎症反应,其机制可能与调控TGF-β表达、抑制炎症相关因子TNF-α、IL-1β、PGE_(2)、NGF有关。 Objective To observe theeffects of Xiangbitonggranule(XBTG)on myocyte morphology and TGF-β,TNF-α,IL-1β,PGE_(2) and NGF in rats with inflammatory injury of posterior cervical muscle andexplorepossible mechanism.Methods Forty SPF male SD rats were randomly divided into sham operation group(SOG),model group(MG),western medicine group(WMG)and traditional Chinese medicine group(TCMG).The rat model was established by surgical incision and clamping middle segment of posterior cervical muscle by C2-6 spinous process with hemostatic forceps(continuously clamping for 10 s,loosening interval for 10 s and full buckle for 3 times).WMG and TCMG were given celecoxib and XBTG respectively,and SOG and MGnormal saline.HE staining was used to observe myocyte morphological change,and ELISAdetected TGF-β,TNF-α,IL-1β,PGE_(2) and NGF on the 2 nd(before intervention)and after 7 daysof intervention.Results On the 2 nd after modeling(before intervention),muscle color was dark-red and TGF-β,TNF-α,IL-1β,PGE_(2) and NGF increased significantly(P<0.05)in MG compared with SOG.After 7 days of intervention,nuclei decreased,structure was disordered,deformation and edema appeared,and TGF-β,TNF-α,IL-1β,PGE2 and NGF increased substantially(P<0.05)of myocytein MG compared with SOG.Compared with MG,nuclei increased slightly,structure became clear,edema degree reduced,and shape was regular in WMG and TCMG(being more obvious),and TNF-α,IL-1β,PGE_(2) and NGF decreased distinctly(P<0.05),while TGF-βwas no significant difference(P>0.05),but the data decreased.Conclusion XBTG can improve myocyte pathological changes and reduce inflammatory response,and its mechanism may be associated with regulation of TGF-βand inhibition of TNF-α,IL-1β,PGE_(2) and NGF.
