老年慢性心力衰竭患者姑息照护质量现状及影响因素分析

Status quo and influencing factors of palliative care quality in elderly patients with chronic heart failure

目的:探讨老年慢性心力衰竭患者的姑息照护质量现状,并对影响老年慢性心力衰竭患者姑息照护质量的相关因素进行分析。方法:采用便利抽样法,选取2021年1-12月商丘市第一人民医院心内科收治的226例老年慢性心力衰竭患者为研究对象,采用一般资料问卷、姑息照护结局量表及Memorial心力衰竭症状评估量表进行调查。共发放问卷226份,回收有效问卷216份,有效回收率为95.58%。结果:216例老年慢性心力衰竭患者的姑息照护质量评分为(17.84±4.37)分,症状负担评分为(40.27±15.95)分;老年慢性心力衰竭患者姑息照护结局量表总分及各维度评分与Memorial心力衰竭症状评估量表总分及各维度评分均呈正相关(均 P<0.01);多重线性回归分析结果显示,居住方式、心功能分级和心力衰竭症状负担是老年慢性心力衰竭患者姑息照护质量的主要影响因素( P<0.01)。 结论:老年慢性心力衰竭患者的姑息照护质量总体处于较低水平,居住方式、心功能分级和症状负担是患者姑息照护质量的主要影响因素,建议临床护理工作者根据影响因素采取相应护理措施。

Objective To explore the current status of palliative care quality in elderly patients with chronic heart failure and to analyze the related factors affecting the quality of palliative care in elderly patients with chronic heart failure.Methods Using the convenient sampling method,a total of 226 elderly patients with chronic heart failure who were admitted to Department of Cardiology in Shangqiu First People's Hospital from January to December 2021 were selected as the research objects.The general information questionnaire,Palliative Care Outcome Scale and Memorial Heart Failure Symptom Assessment Scale were used for investigation.A total of 226 questionnaires were distributed and 216 valid questionnaires were recovered,with an effective recovery rate of 95.58%.Results The palliative care quality score of 216 elderly patients with chronic heart failure was(17.84±4.37),and the symptom burden score was(40.27±15.95).The total score and each dimension score of Palliative Care Outcome Scale of elderly patients with chronic heart failure were all positively correlated with the total score and each dimension score of Memorial Heart Failure Symptom Assessment Scale(all P<0.01).The results of multiple linear regression analysis showed that living style,heart function classification and heart failure symptom burden were the main influencing factors of palliative care quality in elderly patients with chronic heart failure(P<0.01).Conclusions The quality of palliative care for elderly patients with chronic heart failure is generally at a low level.Living style,cardiac function classification and symptom burden are the main influencing factors of the quality of palliative care for patients.It is suggested that clinical nursing workers take corresponding nursing measures according to the influencing factors.
