前馈控制流程对全麻侧卧位手术后并发症及麻醉复苏的影响

Effect of feedforward control process on postoperative complications and anesthesia resuscitation after general anesthesia in lateral position

目的:探讨前馈控制流程对全麻侧卧位手术后并发症及麻醉复苏效果的影响。方法:采用便利抽样法,选取2018年6月-2021年5月南京医科大学附属淮安第一医院就诊的全麻侧卧位手术患者102例,利用随机数字表法将患者分为常规组、前馈组各51例,常规组给予常规围术期管理,前馈组在常规围术期管理基础上引入前馈控制理论护理干预,比较两组患者术后并发症发生情况及麻醉复苏效果。结果:前馈组患者术后体位相关并发症发生率3.92%(2/51)、眼部相关并发症发生率1.96%(1/51),均低于常规组,前馈组麻醉复苏期不良反应发生率低于常规组,差异均有统计学意义( P<0.05);前馈组患者拔管时间、离开复苏室时间短于常规组,差异有统计学意义( P<0.05);两组患者苏醒时间比较差异无统计学意义( P>0.05)。 结论:前馈控制理论护理干预能够降低全麻侧卧位手术患者术后体位及眼部相关并发症发生率,缩短术后拔管时间和转入普通病房时间,麻醉复苏期不良反应发生风险更低,但未缩短患者苏醒时间。

Objective To explore the effect of feedforward control process on postoperative complications and anesthesia recovery.Methods From June 2018 to May 2021,a total of 102 patients undergoing general anesthesia in lateral position in the Affiliated Huai'an No.1 People's Hospital of Nanjing Medical University were enrolled by convenient sampling method,and divided into the conventional group and the feedforward group using random digital table method with 51 cases each.The conventional group utilized the conventional perioperative management,and the feedforward group conducted the feedforward control theoretical care intervention on the basis of conventional perioperative management.The occurrence of postoperative complications and the anesthesia recovery effect were compared between the two groups.Results The occurrence of postoperative posture-related complications,eye-related complications of the feedforward group were 3.92%(2/51),1.96%(1/51)respectively,and they were lower than those of the conventional group with significant differences(P<0.05).The adverse reactions during anesthesia resuscitation of the feedforward group were lower than those of the control group with significant differences(P<0.05).The time of extubation and leaving resuscitation room of the feedforward group were shorter than those of the conventional group,and the differences were statistically significant(P<0.05).There was no difference in the recovery time between the two groups(P>0.05).Conclusions Feedforward control theory nursing intervention can reduce the occurrence of postoperative posture-related complications,eye-related complications and the risk of adverse reactions during anesthesia resuscitation in patients undergoing general anesthesia in lateral position,shorten the time of postoperative extubation and transfer to the general ward,but it has no effect on the time of anesthesia recovery.
