隐睾相关精原细胞瘤的CT影像学表现

CT Imaging Findings of Seminoma Associated with Cryptorchidism

目的 探讨隐睾相关精原细胞瘤的CT影像学表现,以提高对精原细胞瘤的诊断水平。方法 回顾性分析11例隐睾患者经手术或穿刺病理证实的精原细胞瘤CT影像学资料,观察肿瘤部位、大小、形态、边缘、CT值等特征,并与术后病理进行对照。结果 11例隐睾患者,1例双侧隐睾,6例左侧隐睾,4例右侧隐睾;其中6例为腹腔内型隐睾,5例为腹腔外型隐睾。精原细胞瘤呈轻-中度强化,平扫CT值(45.97±6.07)HU,动脉期CT值(57.00±10.00)HU,静脉期CT值(67.24±8.99)HU。以肿瘤直径分组,最大直径≤8 cm和直径>8 cm时,CT值存在差异;按照肿瘤内部有无坏死分组,发现坏死组中实性成分的平扫CT值更高。结论 隐睾相关精原细胞瘤具有一定的CT影像学特征表现,男性隐睾史尤为重要,更有助于提高肿瘤影像诊断准确率。

Objective To investigate the CT imaging features of seminoma associated with cryptorchidism in order to improve the diagnostic level of seminoma.Methods CT imaging findings of 11 patients with cryptorchidism confirmed by operation or biopsy pathology were analyzed retrospectively.The characteristics of tumor location,size,shape,edge and CT value were observed and compared with postoperative pathology.Results There were 11 patients with cryptorchidism,1 bilateral,6 left and 4 right(6 intra-abdominal cryptorchidism and 5 extra abdominal cryptorchidism).Seminoma presented mild to moderate enhancement,with plain CT value of(45.97±6.07)HU,arterial CT value of(57.00±10.00)HU,venous CT value of(67.24±8.99)HU.Grouped by tumor diameter,CT values were different when the maximum diameter≤8 cm and the diameter>8 cm.According to whether there is necrosis in the tumor,the CT plain scan value of solid component in necrotic group was higher.Conclusion Cryptorchid-associated seminoma has certain CT imaging features,especially the history of male cryptorchidism,which is helpful to improve the accuracy of tumor imaging diagnosis.
