“三师共管”模式在助理全科住院医师规范化培训中的应用效果研究

Application of the "three in one model" tutorial system in standardized residency training of general practice

目的分析“三师共管”模式在助理全科住院医师规范化培训(简称住培)中的应用效果与价值。方法选取61名参加助理全科住培的住院医师作为研究对象。2018年、2019年招收的31名住院医师设为观察组,采用“三师共管”模式;2015年、2016年、2017年招收的30名住院医师设为对照组,采用常规培训模式。对比两组培训前后的全科接诊能力、家庭病床管理能力、实践操作能力、公共卫生实践知识评分,以及住院医师对培训导师的满意程度与结业考核通过情况。结果培训后观察组全科接诊能力、家庭病床管理能力、实践操作能力、公共卫生实践知识评分均高出对照组,差异有统计学意义(P<0.05);观察组对培训内容、态度、方法和效果的满意度分别是93.55%、100%、100%、93.55%,均高于对照组的53.33%、56.67%、60%、53.33%,差异有统计学意义(P<0.05)。结论“三师共管”模式可以提高住院医师的接诊与实践操作能力,提高住院医师的满意度,对于促进住院医师综合能力、助理全科医师规范培训中具有积极意义,能够为临床工作培养更多的优质综合型人才。

Objective To analyze the application effect and value of three division co-management model in the standardized training of assistant general practitioners.Methods 61 residents who participated in the standardized training of assistant general residents in our hospital were selected as the research subjects.The 31 residents enrolled in 2018 and 2019 were set as the observation group,adopting the mode of three divisions’comanagement.The 30 residents recruited in 2015,2016 and 2017 were set as the control group,using the routine training mode.Before and after the training,the general practice admitting ability,family hospital bed management ability,practical operation ability,public health practical knowledge score,residents’satisfaction with the trainers and passing the completion examination were compared between the two groups.Results after training,the scores of general department admitting ability,family hospital bed management ability,practical operation ability and public health practical knowledge in the observation group were higher than those in the control group(P<0.05).The satisfaction of training content,attitude,method and effect in the observation group were 93.55%,100%,100%and 93.55%,respectively,which were higher than 53.33%,56.67%,60%and 53.33%in the control group(P<0.05).Conclusion The three division co-management model can improve the residents’admitting and practical operation ability,improve the residents’satisfaction,have positive significance in promoting the residents’comprehensive ability and the standardized training of assistant general practitioners,and can cultivate more highquality comprehensive talents for clinical work.
