医院交互质量、调节焦点与主诊医师负责制决策满意间的关系研究

Research of the Relationship between Interaction Quality,Regulatory Focus and Satisfaction of Attending Physician Responsibility System in Hospital

摘要 目的了解医护人员对主诊医师负责制的决策满意水平以及调节焦点在医院交互质量与决策满意间的中介作用。方法以方便抽样的方法抽取重庆市某三级公立医院234名医护人员,采用交互质量量表、工作调节焦点量表和决策满意量表对其进行调研,并通过相关性分析、结构方程模型和Bootstrap法对数据进行相关性检验、模型构建以及中介效应分析。结果交互质量、促进焦点、防御焦点与决策满意呈正相关(r=0.557,P<0.001;r=0.400,P<0.001;r=0.443,P<0.001),促进焦点、防御焦点在交互质量与决策满意之间具有中介效应(16.0%;18.2%)。结论主诊医师负责制决策满意程度及组织交互质量水平整体较高,在主诊医师负责制实施的过程中,工作调节焦点能够改善交互质量与决策满意之间的关系,并可以通过提高交互质量影响医务人员对主诊医师负责制的态度及行为。 Objective To understand the satisfaction of medical staff with the responsibility system of attending physician and the mediating role of regulatory focus between the interaction quality and the satisfaction of decision-making.Methods A total of 300(78%)health care workers from a third-level public hospital in Chongqing were selected by convenient sampling method and investigated with the interactive quality scale,work regulatory focus scale and decision satisfaction scale.Correlation,model construction and intermediary effect were studied through the correlation analysis,structural equation modeling and bootstrap method.Results Interaction quality,promotion focus and prevention focus were positively correlated with decision satisfaction(r=0.557,P<0.001;r=0.400,P<0.001;r=0.443,P<0.001).Promotion focus and prevention focus played an intermediary role between interaction quality and decision satisfaction(16.0%;18.2%).Conclusion The degree of satisfaction with decision-making and the level of organizational interaction quality are higher in the system of attending physician responsibility.In the process of implementing the system of attending physician responsibility,the focus of work regulation can improve the relationship between interaction quality and decision-making satisfaction,and it can influence the medical staff's attitude and behavior towards the attending physician's responsibility system by improving the interaction quality.
