胶体敷料代替薄膜敷贴固定静脉留置针对输注脂肪乳发生静脉炎的疗效观察

Observation on the curative effect of gel dressing instead of thin-film dressing for fixed vein indwelling against fat emulsion infusion

目的探讨胶体敷料代替薄膜敷贴固定静脉留置针对输注脂肪乳发生静脉炎的疗效。方法以中南大学湘雅医学院附属海口医院2018年5月—2020年6月83例脂肪乳输注患者为研究对象,随机分为对照组42例和观察组41例。对照组采用薄膜敷贴固定静脉留置针方式输注,观察组采用胶体敷料固定静脉留置针方式输注。对比两组静脉炎发生情况、静脉留置针留置时间、疼痛程度、并发症发生率。结果观察组静脉炎发生率(21.43%)明显低于对照组(4.88%)。观察组静脉留置针留置时间(54.26±9.85)h与对照组(42.13±10.56)h相比明显较长(P<0.05)。观察组总体疼痛程度明显高于对照组(χ^(2)=7.232,P=0.027)。观察组并发症发生率(4.88%)与对照组(23.81%)相比明显较低(P<0.05)。结论胶体敷料代替薄膜敷贴固定静脉留置针能够明显降低脂肪乳输注患者静脉炎发生率、疼痛程度、并发症发生率,并提高患者静脉留置针留置时间。

Objective To investigate the effect of colloid dressing instead of film dressing in the treatment of phlebitis caused by fat emulsion infusion.Methods 83 patients with underlying nephritis or rheumatism suffering from phlebitis caused by fat emulsion infusion were randomLy divided into 2 groups:control group(n=42)undergoing infusion of fat emulsion infusion with venous indwelling needle fixed by film dressing and observation group(n=41)undergoing infusion of fat emulsion infusion with venous indwelling needle fixed by colloid dressing.The fixing material was changed every 3 days normally or at any time when there were oozing or loosening.The occurrence of phlebitis,indwelling time of the venous indwelling needle,degree of pain,and complications of the two groups were compared.Results The incidence of phlebitis in the observation group was 21.43%,significantly lower than that in the control group(4.88%,P<0.05).The indwelling time of intravenous indwelling needle in the observation group was(54.26±9.85)h,which was significantly longer than that in the control group[(42.13±10.56)h,P<0.05].The overall degree of pain in the observation group was significantly higher than that in the control group(χ^(2)=7.232,P=0.027).The incidence of complications in the observation group was 4.88%,significantly lower than that of the control group(23.81%,P<0.05).Conclusion Colloidal dressing instead of film dressing to fix the intravenous indwelling needle significantly reduced the incidence of phlebitis,pain degree and complication rate in patients with fat emulsion infusion,and increased the indwelling time of intravenous indwelling needle in patients.
