自发性水平眼震强度在急性孤立性眩晕疾病中的定位诊断意义

The significance of the strength grade of Spontaneous horizontal nystagmus in topical diagnosis of solitary vertigo

目的 探讨自发性水平眼震的强度等级在急性孤立性眩晕疾病中的定位诊断意义.方法 回顾性分析2015年1月-2020年12月于北京怀柔医院神经内科住院的634例孤立性眩晕患者,眼震电图记录患者发病后72小时内自发性水平眼震的方向、强度等级,按眼震强度,分为三组,Ⅰ°自发性水平眼震组206人;Ⅱ°自发性水平眼震组217人,Ⅲ°自发性水平眼震组211人,总计归纳眼震强度与前庭周围性眩晕和前庭中枢性眩晕的定位关联性.结果 前庭周围性疾病在I°自发性水平眼震组占比为73.79%,在Ⅱ°自发性水平眼震组占比为92.17%,在Ⅲ°自发性水平眼震组占比为93.84%,眼震强度越高,前庭周围性病变患者所占比例越高,差异有统计学意义(P<0.05).结论 对于孤立性眩晕患者,自发性水平眼震强度越大,提示前庭周围性疾病可能性越大.

Objective To investigate the significance of the strength grade of Spontaneous horizontal nystagmus in topical diagnosis of acute solitary vertigo.Methods A retrospective analysis was made about 634 patients with solitary vertigo who were hospitalized in Department of Neurology,Hospital of Beijing Huairou district from January 2015 to December 2020.All the patients took the Electronystagmography after onset within 72 hours,the direction and strength grade of nystagmus was recorded.According to the strength grade of nystagmus,they were divided into 3 groups.The first group with Ⅰ°spontaneous horizontal nystagmus,206 patients.The second group with Ⅱ°spontaneous horizontal nystagmus,217 patients.The third group with Ⅲ°spontaneous horizontal nystagmus,211 patients.To find out whether it had the relevance in location between central vestibular disease and perivestibular disease.Resul ts The percentage of perivestibular disease in Ⅰ°spontaneous horizontal nystagmus group was 73.79%,in Ⅱ°spontaneous horizontal nystagmus group was 92.17%,and in Ⅲ°spontaneous horizontal nystagmus group was 93.84%,the greater the nystagmus intensity,the greater proportion of the perivestibular disease,the difference was statistically significant(P<0.05).Conc lusion For patients with solitary vertigo,the greater the spontaneous horizontal nystagmus intensity,the more possibility of the perivestibular disease.
