督灸联合柳氮磺吡啶治疗早中期强直性脊柱炎疗效观察

Observations on the Efficacy of Governor Vessel Moxibustion and Sulfasalazine for Ankylosing Spondylitis in Early and Middle Stage

目的观察督脉铺灸结合柳氮磺吡啶治疗早中期强直性脊柱炎(AS)的临床疗效及其对患者外周血微小RNA-223(miR-223)、基质金属蛋白酶-3(MMP-3)和白介素-6(IL-6)水平及骨代谢指标的影响。方法将符合纳入标准的60例早中期AS患者按随机数字表法分为观察组和对照组,每组30例。对照组予口服柳氮磺吡啶治疗,观察组在对照组治疗基础上予督脉铺灸治疗。比较两组临床疗效。观察两组外周血血清mi R-223、MMP-3和IL-6水平及骨代谢指标[骨钙素(BGP)和骨特异性碱性磷酸酶(BALP)]的变化。结果观察组总有效率为90.0%,高于对照组的73.3%,差异具有统计学意义(P<0.05)。两组治疗后外周血血清miR-223、MMP-3和IL-6水平均低于同组治疗前(P<0.05),骨代谢指标BGP和BALP水平高于治疗前(P<0.05)。治疗后组间比较,观察组外周血血清mi R-223、MMP-3和IL-6及骨代谢指标均优于对照组(P<0.05)。结论督脉铺灸联合柳氮磺吡啶治疗AS疗效优于单纯柳氮磺吡啶治疗,可能与其能降低外周血mi R-223、MMP-3和IL-6水平以及调节骨代谢有关。

Objective To observe the clinical efficacy of Governor Vessel spreading moxibustion plus Sulfasalazine for ankylosing spondylitis(AS)in early and middle stage,and its effect on peripheral blood microRNA-223(miR-223),matrix metalloproteinase-3(MMP-3)and interleukin-6(IL-6)levels,and bone metabolism indicators in the patients.Method Sixty patients with AS in early and middle stage meeting the inclusion criteria were allocated,using a random number table,to an observation group and a control group,with 30 cases in each group.The control group received oral administration of Sulfasalazine and the observation group received Governor Vessel spreading moxibustion in addition.The clinical therapeutic effects were compared between the two groups.Changes in peripheral blood serum miR-223,MMP-3 and IL-6 levels and bone metabolism indicators(bone-γ-carboxyglutamic acid-containing protein(BGP)and bone specific alkaline phosphatase(BALP))were observed in the two groups.Result The total efficacy rate was 90.0% in the treatment group,which was higher than 73.3% in the control group with a statistically significant difference(P<0.05).After treatment,peripheral blood serum miR-223,MMP-3 and IL-6 levels decreased and bone metabolism indicators BGP and BALP levels increased in both groups compared with before(P<0.05).After treatment,peripheral blood serum miR-223,MMP-3 and IL-6 levels and bone metabolism indicators were better in the observation group than in the control group(P<0.05).Conclusion Governor Vessel spreading moxibustion plus Sulfasalazine is more effective than Sulfasalazine alone in treating AS,which may be related to its reducing peripheral blood miR-223,MMP-3 and IL-6 levels and regulating bone metabolism.
