摘要
糖尿病是世纪之病,累及多个系统和多个器官,医疗资源消耗大,有相当一部分糖尿病患者发展为糖尿病足。我国糖尿病发病率高的原因除基因易感之外,与人们生活方式和生活水平的变化有密切的关联。对糖尿病的治疗重在预防,建立健康的生活方式,对易感人群进行长期的监控和对儿童、青少年进行体重控制,开展全民性的糖尿病防控、健康运动是必要的。血循环重建、外科清创、创面修复及矫形骨科治疗等外科手段对糖尿病足的治疗有良好的效果。糖尿病足的治疗过程中要避免风险因素的发生。生物制剂富血小板血浆、外泌体对创面的愈合有一定促进作用,干细胞技术将是未来治疗糖尿病的根本性方案。建立层次分明的糖尿病足管理体系,可以使糖尿病的医疗逐步走向规范和完整。人工智能和再生医学的发展,将给糖尿病及糖尿病足的预防与治疗带来革命性的变化。
Diabetes is a disease of the century,involving multiple systems and organs,with a large consumption of the medical resources.A considerable number of diabetics develop diabetic foot.In addition to genetic susceptibility,the high incidence of diabetes in Chinese people is closely related to changes in people′s lifestyle and living standards.The treatment of diabetes should be focused on prevention,the establishment of a healthy lifestyle,the long-term monitoring of the susceptible people,and the weight control of children and teenagers.It is necessary to carry out nationwide diabetes prevention and control and health campaigns.Surgical methods such as blood circulation reconstruction,surgical debridement,wound repair and orthopaedic treatment have good effects on the treatment of diabetic foot.Risk factors should be avoided in the treatment of diabetic foot.Biological agents such as platelet-rich plasma and exosomes have a certain role in promoting wound healing,and stem cell technology will be the fundamental solution for the treatment of diabetes in the future.The establishment of a well-defined diabetic foot management system can gradually make the medical treatment of diabetes more standardized and complete.The development of artificial intelligence and regenerative medicine will bring revolutionary changes to the prevention and treatment of diabetes and diabetic foot.
