应用正清风痛宁注射液联合针刀治疗老年肩袖损伤合并肩周炎的临床分析

Clinical analysis of Zhengqing Fengtongning injection combined with acupotomy in the treatment of elderly patients with rotator cuff injury complicated with periarthritis of shoulder

目的探讨正清风痛宁注射液联合针刀治疗对老年肩袖损伤合并肩周炎患者的临床疗效,为优化临床治疗方案提供依据。方法选取2021年7月—2022年7月南京医科大学第四附属医院疼痛科收治的老年肩袖损伤伴肩周炎患者共120例纳入此次研究,依照治疗方法的不同将患者分为观察组和对照组,每组60例。对照组患者采用针刀治疗,观察组在针刀治疗基础上加用正清风痛宁注射治疗。观察并比较2组治疗前后的肩关节功能评分、疼痛程度评分、肩关节活动度、生活质量评分以及治疗后的临床疗效。结果治疗前,2组的UCLA评分、VAS评分、肩关节(前屈、外展、外旋)活动度以及SF-36评分差异均无统计学意义(P>0.05);治疗后,2组VAS评分均低于同组治疗前,且观察组VAS评分低于对照组[(4.77±1.75)分vs(2.86±1.02)分)];2组UCLA评分、肩关节活动度、SF-36评分均高于同组治疗前,且观察组上述指标值均高于对照组,差异均有统计学意义(P<0.05)。观察组治疗总有效率明显高于对照组(96.67%vs 90.00%)(P<0.05)。结论正清风痛宁注射液联合针刀治疗可有效改善老年肩袖损伤合并肩周炎患者的肩关节活动度,提高肩关节功能,同时缓解疼痛,提高临床疗效,具有一定的临床应用价值。

Objective To investigate the clinical efficacy of Zhengqing Fengtongning injection combined with acupotomy in the treatment of elderly patients with rotator cuff injury complicated with periarthritis of shoulder,so as to provide clinical basis for the optimization of clinical treatment plan.Methods A total of 120 elderly patients with rotator cuff injury and periarthritis of shoulder admitted to Pain Department of the Fourth Affiliated Hospital of Nanjing Medical University from July 2021 to July 2022 were included in this study.According to the different treatment methods,the patients were divided into control group and observation group,with 60 cases in each group.The control group was treated with acupotomy,and the observation group was treated with ZhengqingFengtongning injection on the basis of acupotomy.The scores of shoulder joint function,pain degree,motion range of shoulder joint and quality of life before and after treatment and clinical efficacy of the two groups were observed and compared.Results Before treatment,there were no significant differences in UCLA score,VAS score,motion range of shoulder joint(forward flexion,abduction,external rotation)and SF-36 score between the two groups(P>0.05).After treatment,the VAS score of both groups was lower than that of the same group before treatment,and the VAS score of the observation group was lower than that of the control group[(4.77±1.75)points vs(2.86±1.02)points];UCLA score,motion range of shoulder joint,SF-36 score of both groups were higher than those of the same group before treatment,and the above indexes of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group(96.67%vs 90.00%)(P<0.05).Conclusion Zhengqing Fengtongning injection combined with acupotomy can effectively improve the motion range of shoulder joint of elderly patients with rotator cuff injury complicated with periarthritis of shoulder,improve shoulder joint function,relieve pain,and improve clinical efficacy,and it has certain clinical application value.
