补体C3、C4水平联合血清白球蛋白比值在系统性红斑狼疮早期诊断中的临床意义

Clinical significance of complements C3 and C4 levels combined with serum albumin/globulin ratio in early diagnosis of systemic lupus erythematosus

目的 研究分析在早期诊断系统性红斑狼疮(SLE)患者期间采用检测补体C3、C4水平联合血清白球蛋白比值(AGR)的作用。方法 选取2019年1月至2020年12月广东省东莞市大朗医院(我院)治疗的100例SLE患者作为观察组,根据有无发生狼疮性肾炎(LN)分LN组42例,非LN组58例,并选取同期体检健康者100例作为对照组,开展回顾性分析,比较两组补体C3、C4水平及AGR水平。结果 观察组患者补体C3、C4指标、血清白蛋白(Alb)、AGR水平低于对照组,差异有统计学意义(P <0.05),血清球蛋白(Glb)指标高于对照组,差异有统计学意义(P <0.05)。LN组Alb、AGR低于非LN组,差异有统计学意义(P <0.05),两组补体C3、C4、Glb水平比较,差异无统计学意义(P> 0.05)。结论 临床上进行补体C3、C4水平联合AGR检测能为SLE患者早期诊断提供支持;AGR与患者狼疮性肾损害有关;本研究并未发现Glb及补体C3、C4水平与狼疮性肾损害有关。

Objective To study and analyze the effect of detecting the complements C3 and C4 levels combined with the serum albumin/globulin ratio(AGR) during the early diagnosis of systemic lupus erythematosus. Methods A total of 100 patients with systemic lupus erythematosus treated in Dalang Hospital of Dongguan(our hospital) from January 2019 to December 2020 were selected as the observation group. According to whether lupus nephritis(LN) occurred or not, they were divided into 42 cases in the LN group and 58 cases in the nonLN group. 100 healthy individuals undergoing physical examination in the same period were selected as the control group. The levels of complements C3, C4 and AGR were compared between the two groups. Results The levels of complements C3, C4, serum albumin(Alb) and AGR in the observation group were lower than those in the control group, with statistically significant differences(P < 0.05), while the serum globulin(Glb) level was higher than that in the control group, with statistically significant difference(P < 0.05). The Alb level and AGR in the LN group were lower than those in the non-LN group, with statistically significant differences(P < 0.05). There was no significant difference in the levels of complement C3, C4 and Glb between the two groups(P > 0.05). Conclusion Detection of the complements C3 and C4 levels combined with AGR in clinical practice can provide support for the early diagnosis of systemic lupus erythematosus. AGR was correlated with lupus nephritis for patients. A correlation between Glb or complements C3 and C4 levels and lupus nephritis has not been discovered in the study.
