早期功能锻炼对乳腺癌术后患者肩关节功能的影响 被引量:2

Effect of early functional exercise on shoulder function in patients with breast cancer after operation

摘要 目的探讨早期功能锻炼对乳腺癌术后患者肩关节功能的影响。方法依据术后开始功能锻炼的时间将87例乳腺癌患者分为常规组(n=41)和早期组(n=46),常规组患者术后14天开始功能锻炼,早期组患者术后1天开始功能锻炼。比较两组患者的患侧肩关节活动度、Constant-Murley肩关节功能评分和并发症发生情况。结果术后1、3个月,两组患者后伸、前屈、外展、内旋、外旋角度均先降低后升高,且早期组患者后伸、前屈、外展、内旋、外旋角度均大于常规组,差异均有统计学意义(P﹤0.05)。术后1、3个月,两组患者疼痛、日常生活活动、主动活动范围、肌力评分均先降低后升高,且早期组患者疼痛、日常生活活动、主动活动范围、肌力评分均高于常规组,差异均有统计学意义(P﹤0.05)。结论早期功能锻炼可改善乳腺癌术后患者肩关节活动度,恢复患者肩关节功能。 Objective To explore the effect of early functional exercise on shoulder function in patients with breast cancer after operation.Method A total of 87 patients with breast cancer were divided into routine group(n=41)and early group(n=46)according to the time of starting functional exercise.Patients in the routine group began functional exercise 14 days after surgery,and patients in the early group began functional exercise 1 day after surgery.The range of motion of affected shoulder joint,Constant-Murley shoulder joint function score and complications were compared between the two groups.Result At 1 and 3 months after operation,the angles of backward extension,forward flexion,abduction,internal rotation and external rotation in the two groups decreased first and then increased,and the angles of backward extension,forward flexion,abduction,internal rotation and external rotation in the early group were greater than those in the routine group,and the differences were statistically significant(P<0.05).At 1 and 3 months after the operation,the pain,activities of daily living,active range of motion,and muscle strength scores in both groups decreased first and then increased,and these scores in the early group were higher than those in the routine group,and the differences were statistically significant(P<0.05).Conclusion Early functional exercise could improve the range of motion of the shoulder joint in patients after breast cancer surgery and promote the recovery of their shoulder joint function.
