反晕征为特征的鹦鹉热衣原体肺炎临床分析 被引量:1

摘要 目的3例患有鹦鹉热衣原体肺炎患者视为本研究观察者,对其特征性影像学特点及临床特征展开分析,进而增加对此疾病的影像学的认识。方法选取北京大学首钢医院呼吸科在2021年1月至2022年8月所治疗的鹦鹉热衣原体肺炎3例患者,回顾其临床资料及胸部CT影像,分析此疾病患者在影像学上的特征性改变。结果患者临床症状主要是发热以及干咳,同时都有鸟类接触史,鸟类亦多有患病,通过对患者采取宏基因二代测序技术获得鹦鹉热衣原体的存在信息,综合分析进而确诊。分析这3例患者的胸部影像学特点,均发现初期存在反晕征这一特征性影像,且在治疗后均有明显吸收好转。结论反晕征作为一项特征性影像学改变,曾在多种疾病当中有相关报道,但对于鹦鹉热衣原体这一疾病的报道较少,仅局限于个案报道。通过对这3例鹦鹉热衣原体的患者影像学进行分析,提高对社区获得性肺炎患者的影像学诊断的能力。 Objective Three patients with chlamydia psittaci pneumonia were regarded as observers in this study,and their characteristic features and clinical features were analyzed,so as to improve the understanding of imaging of this disease.Methods Three patients with chlamydia psittaci pneumonia who were treated in the pulmonary and critical care medicine department of Peking University Shougang hospital from January 2021 to August 2022 were selected.Their clinical data and chest CT images were reviewed to analyze the imaging characteristics of the patients with this disease.Results The main clinical symptoms of the patients were fever and dry cough.At the same time,all the patients had a history of contact with birds,and most of the birds were affected.The existence information of chlamydia pinii was obtained by metagenomic next-generation sequencing technology,and then the diagnosis was made by comprehensive analysis.By analyzing the chest imaging features of these 3 patients,it was found that the characteristic image of reversed halo sign existed in the initial stage,and the absorption improved significantly after treatment.Conclusions As a relatively characteristic imaging change,reversed halo sign has been reported in a variety of diseases,but only few chlamydia psittaci has only been reported.Through imaging analysis of these 3 cases of chlamydia psittaci,the ability of imaging diagnosis of patients with community-acquired pneumonia was improved.
