帕金森病患者炎症介质与认知障碍进程相关性研究

摘要 目的探讨帕金森病(PD)患者炎症介质与认知障碍进程相关性。方法收集PD患者的临床资料并予追溯分析,依据其认知障碍严重程度,将患者分为认知正常组,轻度认知障碍组及重度认知障碍组;同时测定患者炎症介质及其他危险因素。结果PD认知障碍患者C反应蛋白(CRP)、白细胞介素(IL)-6水平均高于认知正常者,差异有统计学意义(P<0.05)。重度认知障碍组CRP、血清淀粉样蛋白A(SAA)水平高于轻度认知组,差异均有统计学意义(P<0.05),IL-6、降钙素原(PCT)水平差异无统计学意义(P>0.05)。经多因素Logistic回归分析,CRP是PD认知障碍的独立危险因素。结论不同炎症介质在PD认知障碍的各阶段可能发挥不同作用,高CRP水平可能是PD认知障碍的独立危险因素,可一定程度预测其发病风险。 Objective To investigate the relationship between inflammatory mediators and cognitive impairment in patients with Parkinson′s disease.Methods The clinical data of 114 patients with Parkinson′s disease were retrospectively collected and analyzed.According to the severity of cognitive impairment,the patients were divided into cognitive normal group,mild cognitive impairment group and severe cognitive impairment group.At the same time,inflammatory mediators and other risk factors were determined.Results The levels of C-reactive protein(CRP)and interleukin-6(IL-6)in Parkinson′s disease patients with cognitive impairment were higher than those with normal cognition,and the differences were statistically significant(P<0.05).The levels of CRP and serum amyloid A(SAA)in the severe cognitive impairment group were higher than those in the mild cognitive impairment group,and the differences were statistically significant(P<0.05).The levels of IL-6 and procalcitonin(PCT)were different in the two groups,but with no statistical significance(P>0.05).Multivariate Logistic regression analysis showed that CRP was an independent risk factor for cognitive impairment in Parkinson′s disease.Conclusion Different components of inflammatory mediators may play different roles in different stages of cognitive impairment in Parkinson′s disease.The high CRP level may be an independent risk factor for Parkinson′s disease cognitive impairment and can predict its risk to a certain extent.
