动态增强CT联合MRI检查在肾上腺肿瘤诊断中的效能

Efficacy of dynamic enhanced CT combined with MRI in diagnosis of adrenal tumors

目的:探讨动态增强CT联合磁共振成像(MRI)检查在肾上腺肿瘤诊断中的效能。方法:选取2019年1月至2020年12月该院收治的65例肾上腺肿瘤患者进行前瞻性研究,所有患者均行动态增强CT、MRI检查,并进行病理检查。以病理检查结果为“金标准”,统计腺瘤、非腺瘤患者例数。比较MRI检查与CT检查在腺瘤、非腺瘤各延迟点的廓清率,以及单项检查与联合检查诊断肾上腺肿瘤与非腺瘤的灵敏度、特异度、准确度。结果:病理检查结果显示,65例肾上腺肿瘤患者中,55例为腺瘤,10例为非腺瘤;动态增强CT、MRI扫描中,延迟1、5、7 min时,腺瘤、非腺瘤廓清率均逐渐上升,且腺瘤廓清率均高于非腺瘤,差异有统计学意义(P<0.05);动态增强CT联合MRI检查诊断肾上腺腺瘤与非腺瘤的灵敏度、准确度均高于两者单项检查诊断,差异有统计学意义(P<0.05)。结论:动态增强CT联合MRI检查在肾上腺肿瘤诊断中的效能高于两者单项检查。

Objective:To explore efficacy of dynamic enhanced computed tomography(CT)combined with magnetic resonance imaging(MRI)in diagnosis of adrenal tumors.Methods:A prospective study was conducted on 65 patients with adrenal tumors admitted to the hospital from January 2019 to December 2020.All patients underwent dynamic enhanced CT and MRI examination and pathological examination.Using the pathological examination results as the gold standard,the numbers of adenoma and non-adenoma patients were statistically analyzed.The clearance rates of MRI and CT at each delay point of adenoma and non-adenoma were compared.Further,the sensitivity,specificity and accuracy of single examination and combined examination in the diagnosis of adrenal tumors and non-adenoma were compared.Results:The pathological examination showed that among the 65 patients with adrenal tumors,55 were adenoma cases and 10 were non-adenoma cases.For the dynamic contrast-enhanced CT and MRI,the clearance rates of adenoma and non-adenoma increased gradually at 1,5 and 7 min after delay,and the clearance rate of adenoma was higher than that of non-adenoma,and the differences were statistically significant(P<0.05).The sensitivity and accuracy of dynamic contrastenhanced CT combined with MRI in the diagnosis of adrenal adenoma and non-adenoma were higher than those of single examination(P<0.05).Conclusions:The efficacy of dynamic contrast-enhanced CT combined with MRI in the diagnosis of adrenal tumors is higher than single examination.
