宫颈癌根治术后患者发生盆底功能障碍性疾病的危险因素分析

Multifactorial Analysis of the Risk of Pelvic Floor Dysfunction Disease After Radical Cervical Cancer Surgery

目的 分析宫颈癌患者术后发生盆底功能障碍性疾病(PFD)的危险因素。方法 选取宫颈癌患者130例,均行宫颈癌根治术,术后按照是否发生PFD分为PFD组和非PFD组。收集患者一般资料,单因素及多因素Logistic分析宫颈癌术后发生PFD的危险因素。结果 130例宫颈癌根治术后患者有75例(57.69%)发生PFD。2组BMI、分娩方式、妊娠次数、分化程度、肛门排气时间、术后放疗比较,差异无统计学意义(P>0.05);PFD组年龄≥45岁、分娩次数≥2、绝经、肿瘤直径≥4 cm、术中出血量多、手术时间长、便秘比率高于非PFD组,差异有统计学意义(P<0.05);Logistic分析显示,年龄≥45岁、分娩次数≥2、绝经、肿瘤直径≥4 cm、术中出血量多、手术时间长、便秘均是宫颈癌术后PFD发生的独立危险因素。结论 年龄≥45岁、分娩次数≥2、绝经、肿瘤直径≥4 cm、术中出血量多、手术时间长、便秘均是宫颈癌术后PFD发生的独立危险因素。

Objective To analyze the risk factors of pelvic floor dysfunction disease(PFD) in patients with cervical cancer after surgery.Methods 130 patients with cervical cancer were selected and all of them were treated with radical surgery.They were divided into PFD group and non PFD group according to whether PFD occurred.The general data of patients were collected, and the risk factors of PFD after cervical cancer surgery were analyzed by single factor and multi factor logistic analysis.Results PFD occurred in 75(57.69%) of 130 patients with cervical cancer after radical surgery.There was no significant difference in BMI,mode of delivery, number of pregnancies, degree of differentiation, anal exhaust time and postoperative radiotherapy between the 2 groups(P>0.05);In PFD group, age ≥45 years, delivery times ≥2,menopause, tumor diameter ≥4 cm, more bleeding during operation, longer operation time, constipation rate were higher than those in non PFD group(P<0.05);Logistic analysis showed that age ≥45 years, number of deliveries ≥2,menopause, tumor diameter ≥4 cm, large intraoperative bleeding, long operation time and constipation were independent risk factors for PFD after cervical cancer surgery.Conclusion Age ≥45 years old, delivery times ≥2,menopause, tumor diameter ≥4 cm, large intraoperative bleeding, long operation time, constipation are all independent risk factors for PFD after cervical cancer surgery.
