分化型甲状腺癌患者术后心理弹性的影响因素分析

Influencing Factors of Postoperative Psychological Resilience in Patients with Differentiated Thyroid Cancer

目的研究分化型甲状腺癌(DTC)患者术后心理弹性的相关影响因素及干预策略。方法以濮阳市安阳地区医院2019年1月至2020年12月收治的98例接受手术治疗的DTC患者为研究对象,采用单因素分析影响DTC患者术后心理弹性量表(CD-RISC)评分的因素,对有差异的因素赋值并进行多重线性分析。结果98例DTC患者术后CD-RISC评分为(59.43±7.47)分;单因素分析结果显示,不同家庭人均月收入、术前淋巴结转移、病情了解程度、肿瘤直径、放射性碘治疗、领悟社会支持评定量表(PSSS)评分、疾病复发危险性、华西心情指数量表(HEI)评分DTC患者术后CD-RISC评分比较,差异有统计学意义(P<0.05);多重线性回归分析,术前淋巴结转移、家庭人均月收入、肿瘤直径、疾病复发危险性、病情了解程度、放射性碘治疗、PSSS评分、HEI评分为DTC患者术后CD-RISC评分的影响因素(P<0.05)。结论DTC术后患者心理弹性处于中等程度,HEI评分、PSSS评分、病情了解程度等因素与DTC患者术后CD-RISC量表评分有关,临床可依据上述影响因素对护理措施进行调整,以提高患者心理弹性。

Objective To study the influencing factors and intervention strategies of psychological resilience in patients with differentiated thyroid cancer(DTC).Methods A total of 98 patients with DTC who received surgical treatment from January 2019 to December 2020 in Puyang Anyang District Hospital were selected as the research objects,and the factors affecting postoperative Connor-Davidson resilience scale(CD-RISC)scores of DTC patients were analyzed by univariate analysis,multivariate linear analysis was performed on the assignment of different factors.Results The average postoperative CD-RISC score of 98 DTC patients was(59.43±7.47)points.Univariate analysis showed that postoperative CD-RISC scores of DTC patients with different monthly household income,preoperative lymph node metastasis,degree of disease understanding,tumor diameter,radioactive iodine treatment,(perceived social support scale)PSSS score,risk of disease recurrence and Huaxi emotional-distress index(HEI)score were statistical different(P<0.05).Multiple linear regression analysis showed that preoperative lymph node metastasis,family monthly income per capita,tumor diameter,risk of disease recurrence,degree of disease understanding,radioactive iodine therapy,PSSS score and HEI score were the influencing factors of postoperative CD-RISC score in DTC patients(P<0.05).Conclusion The psychological resilience of patients after DTC is at a moderate level.Factors such as HEI score,PSSS score and degree of understanding of the disease are related to the postoperative CD-RISC score of DTC patients.Clinical nursing measures can be adjusted according to the above influencing factors to improve the patients’condition.
