摘要
目的探究术中体位干预结合泡沫敷料对老年肝胆外科长时间手术患者压力性损伤及凝血状态的影响。方法择取106例老年肝胆外科长时间手术患者作为研究对象,根据护理方案不同将其分为对照组(53例,常规护理)和观察组(53例,常规护理+术中体位干预+泡沫敷料)。比较两组的干预效果。结果观察组的压力性损伤总发生率低于对照组(P<0.05)。干预后,观察组的凝血酶-抗凝血酶复合物(TAT)、血栓调节蛋白(TM)、组织型纤溶酶原激活物抑制剂复合物(t PAIC)水平均低于对照组(P<0.05)。干预后,观察组的Kolcaba舒适状况量表各维度评分均高于对照组(P<0.05)。结论老年肝胆外科长时间手术患者术中给予体位干预结合泡沫敷料,不仅能减少压力性损伤,改善凝血状态,还能提高患者术后舒适度。
Objective To explore the effects of intraoperative postural intervention combined with foam dressing on pressure injury and coagulation status in elderly patients undergoing long term hepatobiliary surgery.Methods A total of106 elderly patients with long term hepatobiliary surgery were selected as the research objects,and the patients were divided into control group(53 cases,routine nursing)and observation group(53 cases,routine nursing+intraoperative posture intervention+foam dressing)according to the different nursing plan.The intervention effects of the two groups were compared.Results The total incidence of pressure injury in the observation group was lower than that in the control group(P<0.05).After intervention,the levels of thrombin-antithrombin complex(TAT),thronmbomodulin(TM)and tissue plasminogen activator inhibitor complex(tPAIC)in the observation group were lower than those in the control group(P<0.05).After intervention,the scores of each dimension of Kolcaba comfort scale in the observation group were higher than those in the control group(P<0.05).Conclusion Intraoperative postural intervention combined with foam dressing can not only reduce stress injury,improve coagulation status,but also improve postoperative comfort of elderly patients undergoing long term hepatobiliary surgery.
