妊娠糖尿病患者血糖情况对分娩方式及不良妊娠结局的影响

Effect of Blood Glucose on Delivery Mode and Adverse Pregnancy Outcome in Patients with Gestational Diabetes Mellitus

目的 分析妊娠糖尿病患者血糖情况对分娩方式及不良妊娠结局的影响。方法 选取2020年5月—2021年5月期间福建省妇幼保健院收治的300例血糖水平控制不良的妊娠糖尿病患者设为观察组;另选取同期本院收治的300例血糖水平控制正常的妊娠糖尿病患者设为对照组。对比分析两组分娩结局。结果观察组足月顺产率为69.67%,低于对照组的足月顺产率82.00%;观察组足月剖宫产率为28.67%,高于对照组的足月剖宫产率17.67%,差异有统计学意义(P<0.05)。两组早产率及难产率比较,差异无统计学意义(P>0.05)。观察组不良妊娠结局发生率为7.67%,明显高于对照组的0.67%,差异有统计学意义(P<0.05)。结论妊娠糖尿病血糖水平控制不良会提升不良妊娠结局发生率及足月剖宫产率,因此对妊娠糖尿病患者需加强对血糖的控制。

Objective To analyze the influence of blood glucose on delivery mode and adverse pregnancy outcome in pregnant women with diabetes.Methods 300 patients with gestational diabetes mellitus with poorly controlled blood glucose levels admitted to Fujian Maternal and Child Health Hospital between May 2020 and May 2021 were selected as the observation group. Another 300 patients with gestational diabetes mellitus with normal glycemic control were selected as the control group, and the delivery outcomes of the two groups were compared and analyzed.Results The full-term spontaneous labor rate of the observation group was 69.67%, which was lower than that of the control group(82.00%);the term cesarean section rate in the observation group was 28.67%, which was higher than that in the control group(17.67%), the difference was statistically significant(P<0.05). There was no statistically significant difference in preterm birth rate and dystocia rate between the two groups(P>0.05). The incidence of adverse pregnancy outcome in the observation group was 7.67%, which was significantly higher than 0.67% of the control group, the difference was statistically significant(P<0.05).Conclusion Poor control of blood glucose level in pregnancy diabetes will increase the incidence of adverse pregnancy outcomes and the rate of full-term cesarean section, so it is necessary to strengthen the control of blood glucose in patients with pregnancy diabetes.
