微切口超声乳化术对糖尿病合并白内障患者眼表的影响

摘要 目的 探究糖尿病合并白内障患者采用微切口超声乳化术对于眼表的影响。方法 选择北京市顺义区医院2020年1月—2021年1月就诊的62例糖尿病合并白内障患者,按照随机数表法进行分为对照组和观察组,各31例。对照组(46眼)采用常规3.0 mm透明角膜切口超声乳化吸除术治疗,观察组(48眼)采用1.8 mm角膜微切口超声乳化吸除术治疗,比较两组糖尿病合并白内障患者的OSDI评分、角膜知觉、泪膜破裂时间、基础泪液分泌试验情况。结果 观察组术后1周,术后1、3、6个月OSDI评分均小于对照组,差异有统计学意义(P<0.05)。观察组术后1周,术后1、3、6个月角膜知觉、BUT各项指标值均大于对照组,差异有统计学意义(P<0.05)。观察组术后1周,术后1、3个月的SIT为(7.27±2.72)、(8.74±3.73)、(9.44±3.22)mm/5 min均大于对照组,差异有统计学意义(t=2.223、2.144、2.173,P<0.05)。结论 糖尿病合并白内障患者应用微切口超声乳化术治疗对眼表的影响更小。 Objective To investigate the effects of microincisional ultrasound emulsification on the ocular surface in patients with diabetic combined cataract.Methods From January 2020 to January 2021, 62 patients with diabetes mellitus and cataract in Shunyi District Hospital of Beijing were selected and randomly divided into the control group and the observation group, with 31 cases in each group. The control group(46 eyes) was treated with conventional 3.0mm transparent corneal incision phacoemulsification, and the observation group(48 eyes) was treated with 1.8 mm micro corneal incision phacoemulsification, OSDI score, corneal perception, tear film rupture time and basic tear secretion test were compared between the two groups of diabetic patients with cataract.Results The scores of OSDI in the observation group were lower than those in the control group at 1 week, 1 month, 3 months and 6 months after operation, the difference was statistically significant(P<0.05). The values of corneal perception, BUT in the observation group at 1 week, 1, 3 and 6 months after operation were higher than those in the control group, the difference was statistically significant(P<0.05). The SIT of the observation group at 1 week, 1 month and 3 months after surgery were(7.27±2.72) mm/5 min,(8.74±3.73) mm/5 min,(9.44±3.22) mm/5 min higher than those of the control group, the difference was statistically significant(t=2.223, 2.144, 2.173, P<0.05).Conclusion The application of microincisional ultrasound phacoemulsification treatment in patients with diabetes combined with cataract has less impact on the ocular surface.
