不同中医辨证分型的糖尿病合并高脂血症患者血糖水平、血脂水平和炎症因子水平的研究

Study on the Level of Blood Glucose,Blood Lipid Levels,and Inflammatory Factor Levels of Different Diabetic Diabetes with Different Chinese Medicine Derailment Type

目的研究不同中医辨证分型的糖尿病合并高脂血症患者血糖水平、血脂水平和炎症因子水平差异和意义。方法随机选择2021年1—12月于武威市中医医院内分泌科门诊就医并住院治疗的糖尿病合并高脂血症患者300例进行临床研究。根据证型不同,将患者分为阴虚热盛证(37例)、气阴两虚证(144例)、阴阳两虚证(18例)、肝肾阴虚证(59例)和脾胃湿热证(42例)。所有研究对象均进行血糖、血脂、白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)检测。比较不同中医辨证分型患者的血糖水平、血脂水平和炎症因子水平。结果脾胃湿热证患者FPG、HbA1c高于其他4种证型,阴阳两虚证患者2 hPG水平高于其他4种证型,差异有统计学意义(P<0.05);5种证型患者TG、LDL、HDL、TC水平差异无统计学意义(P>0.05);阴阳两虚证和气阴两虚证患者IL-6、CRP、TNF-α水平高于其他3种证型,差异有统计学意义(P<0.05)。结论不同中医辨证分型糖尿病合并高脂血症患者的IL-6、CRP、TNF-α等炎性因子水平有差异,其中阴阳两虚证和气阴两虚患者炎性因子水平最高,这种差异既能够为中医辨证提供客观依据,也能够为糖尿病合并高血脂症的中医药防治提供帮助。

Objective To study the differences and significance of blood glucose levels,blood lipid levels,and inflammatory factor levels of diabetes in different Chinese medicine diabetic combined with hyperlipidemia.Methods From January to December 2021,300 patients with diabetes combined with hyperlipidemia who were hospitalized in the endocrine department of Wuwei Traditional Chinese Medicine Hospital were randomly selected for clinical research.According to different syndrome types,the patients were divided into yin deficiency and heat excess syndrome(37 cases),qi and yin deficiency syndrome(144 cases),yin and yang deficiency syndrome(18 cases),liver and kidney yin deficiency syndrome(59 cases)and spleen and stomach damp heat syndrome(42 cases).All subjects received blood glucose,blood lipid,interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor-α(TNF-α)testing.The levels of blood glucose,blood lipid and inflammatory factors in patients with different TCM syndrome differentiation were compared.Results FPG and HbA1c in patients with spleen and stomach damp heat syndrome were higher than those in the other four syndrome types,and 2 hPG levels in patients with yin yang deficiency syndrome were higher than those in the other four syndrome types,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the levels of TG,LDL,HDL and TC among the five syndrome types(P>0.05).The levels of IL-6,CRP and TNF-αin patients with yin and yang deficiency syndrome and qi and yin deficiency syndrome were higher than those in the other three syndrome types,and the difference was statistically significant(P<0.05).Conclusion There are differences in the levels of inflammatory factors such as IL-6,CRP,TNF-αin patients with diabetes combined with hyperlipidemia according to different TCM syndrome differentiation types.Among them,the level of inflammatory factors in patients with yin and yang deficiency syndrome and qi and yin deficiency syndrome is the highest.This difference can not only provide an objective basis for TCM syndrome differentiation,but also provide help for TCM prevention and treatment of diabetes combined with hyperlipidemia.
