摘要
目的:比较64排螺旋CT的多平面重建(multi-plane reconstruction,MPR)与容积再现(volume rendering,VR)技术在肋骨骨折诊断中的应用价值。方法:选取2019年1月—2022年5月于无锡市精神卫生中心经64排螺旋CT肋骨扫描确诊的肋骨骨折患者48例,采用多平面重建与容积再现技术分别对肋骨薄层图像进行图像重组,得到清晰的肋骨三维图像,通过观察肋骨骨折的位置、数量、性质及断端等情况,比较两种技术对肋骨骨折的诊断效能。结果:容积再现图像显示肋骨骨折83处,可疑或者未显示骨折9处。多平面重建图像显示肋骨骨折91处,可疑骨折1处。两种技术比较差异有统计学意义(P<0.05)。所有92处骨折中隐匿性骨折42处,多平面重建图像检出32例(76.19%),容积再现检出19例(45.24%),两者相比,多平面重建对隐匿性肋骨骨折的检出率较高。48例患者中合并胸腔积液3例、肺挫伤3例、气胸1例、锁骨骨折1例、腰椎横突骨折2例,并发症发生率为20.83%。结论:64排螺旋CT对肋骨骨折的诊断和并发症的检出有重要应用价值;多平面重建技术对肋骨骨折的诊断效能优于容积再现技术。
Objective To compare the application value of 64-slice spiral CT multi-plane reconstruction(MPR)and volume rendering(VR)in the diagnosis of rib fracture.Methods A retrospective analysis was conducted on 48 patients with rib fractures diagnosed by 64-slice spiral CT rib scanning in Wuxi Mental Health Center from January 2019 to May 2022.Multiplane reconstruction and volume rendering techniques were used to reconstruct thin layer images of ribs respectively to obtain clear 3D rib images.By observing the position,number,nature and broken end of rib fractures,etc.To compare the diagnostic efficacy of the two techniques for rib fracture.Results Volume rendering images showed 83 rib fractures and 9 suspected or non-revealed fractures.Multi-plane reconstructions showed 91 rib fractures and 1 suspected fracture.The difference between the two techniques was statistically significant(P<0.05).Among all 92 fractures,42 were occult fractures,and 32(76.19%)were detected by multi-plane reconstruction and 19(45.24%)were detected by volume rendering.Compared with the two,the detection rate of occult rib fractures by multi-plane reconstruction was higher.Among the 48 patients,there were 3 cases of pleural effusion,3 cases of lung contusion,1 cases of pneumothorax,1 case of clavicle fracture and 2 cases of lumbar transverse process fracture,with a complication rate of 20.83%.Conclusion 64-slice spiral CT has important application value in the diagnosis and detection of complications of rib fracture.The diagnostic efficiency of multi-plane reconstruction is better than that of volume rendering for rib fracture.
