在妇科腹腔镜手术麻醉中应用七氟醚联合瑞芬太尼的效果观察

Effect Observation of Sevoflurane Combined with Remifentanil in Anesthesia During Gynecological Laparoscopic Surgery

目的:探讨七氟醚联合瑞芬太尼在妇科腹腔镜手术麻醉中的效果。方法:选取2019年9月-2021年9月复旦大学附属中山医院青浦分院妇科接受腹腔镜手术治疗的患者100例为观察对象,随机数字表法分为对照组与研究组,各50例。对照组采取丙泊酚联合瑞芬太尼进行常规麻醉,研究组采取七氟醚联合瑞芬太尼进行麻醉。比较两组患者术后各项麻醉指标、手术期间不同阶段血流动力学指标、不良反应发生情况。结果:研究组患者的意识清醒时间、定向力恢复时间、自主呼吸恢复时间、拔管时间及睁眼时间均短于对照组,差异有统计学意义(P<0.05);研究组患者手术期间不同阶段血流动力学指标明显优于对照组,差异有统计学意义(P<0.05);研究组患者的不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:在妇科腹腔镜手术麻醉中应用七氟醚联合瑞芬太尼麻醉,可改善患者麻醉效果,调节不同时间血流动力学指标及减少不良反应的发生。

Objective: To explore the effect of sevoflurane combined with remifentanil in anesthesia during gynecologicallaparoscopic surgery. Methods: From September 2019 to September 2021, 100 patients who underwent laparoscopic surgery in theDepartment of Gynecology, Qingpu Branch, Zhongshan Hospital, Fudan University were selected as observation objects, and theywere divided into control group and study group by random number table method, with 50 cases in each group. The control groupwas treated with propofol plus remifentanil for routine anesthesia, and the study group was treated with sevoflurane plusremifentanil for anesthesia. The postoperative anesthesia indexes, hemodynamic indexes at different stages during surgery andadverse reactions were compared between the two groups. Results: The consciousness time, orientation recovery time, spontaneousbreathing recovery time, extubation time and eye opening time in the study group were all shorter than those in the control group,and the differences were statistically significant(P<0.05). The hemodynamic indexes of patients in the study group at differentstages during surgery were significantly better than those in the control group, and the difference was statistically significant(P<0.05). The incidence of adverse reactions in the study group was significantly lower than that in the control group, and thedifference was statistically significant(P<0.05). Conclusion: The application of sevoflurane combined with remifentanil foranesthesia in gynecological laparoscopic surgery can improve the anesthesia effect of patients, adjust hemodynamic indexes atdifferent times and reduce the occurrence of adverse reactions.
