甲状腺球蛋白测定在分化型甲状腺癌术后随访的作用

Role of Thyroglobulin Determination in Postoperative Follow-Up of Differentiated Thyroid Cancer

目的:分析甲状腺球蛋白(Tg)测定在分化型甲状腺癌术后随访的作用。方法:选取2020年8月-2021年8月苏州市吴中区尹山湖医院收治的分化型甲状腺癌全切或几乎全切术患者78例为研究对象,术后对患者进行Tg指数测定。根据测定结果,淋巴结转移患者26例作为观察组,未发生淋巴结转移患者52例作为对照组。比较两组患者一般资料、Tg水平、Tg和抗甲状腺球蛋白抗体(TgAb)阳性率。结果:观察组术后12个月Tg水平高于对照组,差异有统计学意义(P<0.05);观察组Tg和TgAb阳性率高于对照组,差异有统计学意义(P<0.05)。结论:TgAb在血清中浓度较低时,Tg对监测分化型甲状腺癌的转移和复发有一定的指导作用。在长期的随访过程中,科学合理地将二者结合,能够进一步加强对患者疗效和预后的判断。

Objective:To analyze the role of thyroglobulin(Tg)determination in postoperative follow-up of differentiated thyroid cancer.Methods:A total of 78 patients who underwent total or almost total resection of differentiated thyroid cancer in Yinshan Lake Hospital in Wuzhong District of Suzhou City from August 2020 to August 2021 were selected as the study subjects,and the Tg index was measured after surgery.According to the determination results,26 cases with lymph node metastasis were used as the observation group,and 52 cases without lymph node metastasis were used as the control group.General data,Tg level,Tg and anti-thyroglobulin antibody(Tg Ab)positive rates were compared between the two groups.Results:The Tg level in the observation group was higher than that in the control group at 12 months after surgery,and the difference was statistically significant(P<0.05).The positive rates of Tg and Tg Ab in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:At a low concentration of Tg Ab in serum,TG plays a guiding role in monitoring the metastasis and recurrence of differentiated thyroid cancer.In the long-term follow-up process,scientific and reasonable combination of the two indexes can further strengthen the judgment of curative effect and prognosis.
