摘要
目的探讨参附注射液对脓毒症急性肾损伤患者容量反应性影响研究。方法入选2018年05月至2019年07月江西省人民医院重症医学科及肾内科收治的脓毒症急性肾损伤患者108例,按计算机产生的随机数字表将患者分为2组:附注射液组(A组,n=56)和安慰剂组(B组,n=52)。以PLR后CI增加≥10%容量反应阳性,<10%为容量反应阴性。将给予活性药物前容量反应性阴性的患者设定为容量反应性阴性亚组(N亚组),即AN亚组和BN亚组。结果AN亚组和BN亚组在给予参附注射液和安慰剂后容量反应阳性分别为67%(16例)和8%(2例)。与给药前比较,AN组容量反应阳性率升高(P<0.01),与BN组比较,AN组容量反应阳性率升高(P<0.01)。结论对于容量反应性阴性的脓毒症急性肾损伤患者,参附注射液能够改善其容量反应性。
Objective Objective to investigate the effect of Shenfu Injection on volume responsiveness in patients with sepsis and acute kidney injury.Methods from May 2018 to July 2019,108 patients with sepsis with acute kidney injury admitted to the Department of critical medicine and Nephrology of Jiangxi Provincial People’s hospital were selected,The patients were randomly divided into two groups:Shenfu injection group(group A,n=56)and placebo group(group B,n=52).An increase in cardiac index(CI)≥10% after PLR was considered to be a positive response,whereas CI<10% after PLR was considered to be a negative respone.Patients with negative volume response before active drug administration were assigned to volume response negative subgroup(N subgroup),that was AN subgroup and BN subgroup.Results The positive rate of volume reaction was 67%(16 cases)in an subgroup and 8%(2 cases)in BN subgroup after administration of Shenfu injection and placebo,respectively.Compared with that before administration,the positive rate of volume reaction in an group increased(P<0.05),Compared with BN group,the positive rate of volume reaction in an group was higher(P<0.05).Conclusion Shenfu injection can improve the volume responsiveness of sepsis patients with acute kidney injury.
