胃肠道间质瘤患者Ki-67和钙激活通道蛋白1表达与预后的关系

摘要 目的分析胃肠道间质瘤(GIST)患者Ki-67和钙激活通道蛋白1(DOG-1)表达与预后的关系。方法对杭州师范大学附属医院2018年1月—2021年12月收治的107例GIST患者的临床资料进行回顾性分析,通过Kaplan-Meier法绘制不同Ki-67和DOG-1表达水平下GIST患者的累积生存率曲线,通过ROC曲线分析Ki-67和DOG-1对GIST患者预后的预测价值。结果Ki-67高表达和DOG-1阳性表达与肿瘤直径、肿瘤转移、核分裂象计数、美国国立卫生研究院(NIH)危险度分级密切相关(均P<0.01);Kaplan-Meier分析显示,Ki-67低表达患者和DOG-1阴性患者累积生存率分别高于高表达患者和阳性患者,差异均有统计学意义(均P<0.05);ROC曲线分析显示,Ki-67和DOG-1表达水平预测GIST预后的AUC分别为0.847和0.940(P<0.01)。结论Ki-67和DOG-1对GIST预后分别具有中等和较高预测效能,可作为重要预后影响因子,结合核分裂象计数和NIH危险度分级综合评估患者预后,可增进患者获益。 Objective To analyze the relationship between Ki-67,calcium-activated channel-1(DOG-1)expression and prognosis of patients with gastrointestinal stromal tumor(GIST).Methods The clinical data of 107 GIST patients admitted to the Affiliated Hospital of Hangzhou Normal University from January 2018 to December 2021 were retrospectively analyzed.The cumulative survival rate of GIST patients with different Ki-67 and DOG-1 expression levels was plotted by Kaplan-Meier method.The value of Ki-67 and DOG-1 in predicting the prognosis of GIST patients was analyzed by ROC curve.Results The high expression of Ki-67 and the positive expression of DOG-1 were closely correlated with tumor diameter,tumor metastasis,mitotic count and NIH risk classification(all P<0.01).Kaplan-Meier analysis showed that the cumulative survival rate of GIST patients was significantly correlated with the expression levels of Ki-67 and DOG-1(all P<0.05).ROC curve analysis showed that the AUC of Ki-67 and DOG-1 expression levels in predicting the prognosis of GIST were 0.847 and 0.940,respectively(P<0.01).Conclusion Ki-67 and DOG-1 have moderate or high predictive efficacy for the prognosis of GIST;combined with mitotic count and NIH risk classification,these two markers may more effectively predict the prognosis of patients.
