毒蕈碱型胆碱能受体M2亚型去整合素金属蛋白酶8及骨膜蛋白与支气管哮喘患儿小气道功能的相关性分析

摘要 目的 分析毒蕈碱型胆碱能受体M2亚型(M2受体)、去整合素金属蛋白酶8(ADAM8)及骨膜蛋白与支气管哮喘患儿小气道功能的相关性。方法 选择2021年3月—2022年3月湖州市中心医院收治的112例支气管哮喘患儿为支气管哮喘组,同时选取通过健康体检的50例儿童为对照组。采用酶联免疫吸附试验测定血清M2受体、骨膜蛋白水平,采用实时荧光定量PCR法测定ADAM8相对表达量,使用常规通气法测定小气道功能相关指标。对比两组各指标表达水平、各指标在支气管哮喘组疾病不同时期、不同严重程度中的表达,并分析M2受体、ADAM8、骨膜蛋白与支气管哮喘患儿小气道功能的相关性。结果 支气管哮喘组M2受体、ADAM8、骨膜蛋白水平[(1.94±0.75)U/ml,(1.64±0.24),(84.65±12.66)μg/L]均高于对照组[(1.31±0.42)U/ml,(0.83±0.11),(70.42±9.12)μg/L],小气道功能指标用力呼气25%流速(FEF25)、用力呼气50%流速(FEF50)、用力呼气75%流速(FEF75)、最大呼气中期流量(MMEF)水平均低于对照组,差异均有统计学意义(均P<0.05)。M2受体、ADAM8、骨膜蛋白水平及小气道功能指标在支气管哮喘患儿疾病不同时期表达差异均有统计学意义(均P<0.05)。支气管哮喘患儿疾病不同严重程度M2受体、ADAM8、骨膜蛋白水平及小气道功能指标表达对比差异均有统计学意义(均P<0.05)。相关性分析发现:支气管哮喘患儿M2受体、ADAM8、骨膜蛋白与小气道功能均呈负相关(均P<0.05)。结论 M2受体、ADAM8及骨膜蛋白在儿童支气管哮喘中均为高表达,且与患儿小气道功能有关,检测M2受体、ADAM8及骨膜蛋白水平有助于评价支气管哮喘患儿小气道功能。 Objective To analyze the correlations of muscarinic cholinergic receptor M2 subtype(M2 receptor), a disintegrin and metalloproteinase 8(ADAM8), periostin and small airway function in children with bronchial asthma. Methods From March 2021 to March 2022, 112 children with bronchial asthma treated in Huzhou Central Hospital were selected as bronchial asthma group, 50 healthy children were selected as control group. ELISA was used to detect serum levels of M2 receptor and periostin, real-time fluorescent quantitative PCR was used to detect the relative expression level of ADAM8, conventional ventilation method was used to detect related indexes of small airway function. The levels of the indexes in the two groups, in different stages and degrees of bronchial asthma were compared, the correlations of M2 receptor, ADAM8, periostin and small airway function in children with bronchial asthma were analyzed. Results The levels of M2 receptor, ADAM8, and periostin in bronchial asthma group [(1.94±0.75)U/ml,(1.64±0.24),(84.65±12.66) μg/L] were statistically significantly higher than those in control group [(1.31±0.42)U/ml,(0.83±0.11),(70.42±9.12) μg/L], forced expiratory flow at 25% of vital capacity(FEF25), forced expiratory flow at 50% of vital capacity(FEF50), forced expiratory flow at 75% of vital capacity(FEF75), maximal mid-expiratory flow(MMEF) in bronchial asthma group were statistically significantly lower than those in control group(all P<0.05). There were statistically significant differences in the levels of M2 receptor, ADAM8, periostin, and small airway function indexes among the children in different stages of bronchial asthma(all P<0.05). There were statistically significant differences in the levels of M2 receptor, ADAM8, periostin, and small airway function indexes among the children in different degrees of bronchial asthma(all P<0.05). Correlation analysis showed that M2 receptor, ADAM8, and periostin in children with bronchial asthma were negatively correlated with small airway function(all P<0.05). Conclusion M2 receptor, ADAM8, and periostin are all highly expressed in children with bronchial asthma, and they are related to small airway function of children.The detection of M2 receptor, ADAM8, and periostin levels is helpful to evaluate small airway function of children with bronchial asthma.
