关节镜联合中药治疗脑卒中后肩关节疼痛的疗效

The Efficacy of Arthroscopy Combined with Traditional Chinese Medicine in the Treatment of Post-stroke Shoulder Pain

目的探讨脑卒中后肩关节疼痛患者采用关节镜微创手术联合中药外敷治疗的临床效果。方法选取2018年9月至2021年9月于东台市中医院治疗的脑卒中后肩关节疼痛患者62例作为研究对象,按照随机数字表法分为对照组和观察组,各31例。对照组予以保守康复治疗,观察组予以关节镜微创手术联合中药外敷,比较两组的治疗效果。结果观察组治疗有效率为96.77%,高于对照组的74.19%,差异有统计学意义(P<0.05)。治疗2周、治疗1个月及治疗3个月,观察组视觉模拟评分法(VAS)评分明显低于对照组,差异有统计学意义(P<0.05)。治疗3个月后,观察组前屈、外旋、外展、内旋关节活动度(ROM)均大于对照组(P<0.05)。治疗3个月后,观察组白细胞介素-1β(IL-1β)水平明显低于对照组(P<0.05)。结论关节镜微创手术联合中药外敷治疗脑卒中后肩关节疼痛临床疗效显著,可促进炎症因子水平下降,改善疼痛症状,促进肩关节功能恢复。

Objective To explore the clinical effect of external application of traditional Chinese medicine in the treatment of post-stroke shoulder pain after arthroscopic minimally invasive surgery.Methods 62 patients with post-stroke shoulder pain who were treated in Dongtai Traditional Chinese Medicine Hospital from September 2018 to September 2021 were selected as research objects,and were divided into control group and observation group according to random number table,with 31 patients in each group.The control group was given conservative rehabilitation treatment,and the observation group was given arthroscopic minimally invasive surgery combined with external application of traditional Chinese medicine.The therapeutic effects of the two groups were compared.Results The effective rate of treatment in the observation group was 96.77%,higher than that in the control group(74.19%),with a statistically significant difference(P<0.05).After 2 weeks,1 month and 3 months of treatment,the VAS score of the observation group was significantly lower than that of the control group(P<0.05).After 3 months of treatment,ROM of flexion,external rotation,abduction and internal rotation in the observation group were higher than those in the control group(P<0.05).After 3 months of treatment,the level of interleukin-1β(IL-1β)in the observation group was significantly lower than that of the control group(P<0.05).Conclusion Arthroscopic minimally invasive surgery combined with external application of traditional Chinese medicine has a significant clinical effect in the treatment of shoulder pain after stroke,which can promote the decrease of inflammatory factor levels,improve pain symptoms,and promote the recovery of shoulder joint function.
