生殖支原体感染与精液质量的相关性研究 被引量:2

摘要 目的:通过对不育男性精液参数的回顾性研究,探讨生殖支原体(MG)对不育男性精液参数的影响,为男性不育的治疗提供理论支持。方法:回顾性分析2019年9月-2021年9月在山东大学附属生殖医院就诊的不育男性12136例,根据SAT技术检测患者尿中MG RNA的结果分为MG阳性组和MG阴性组,西班牙CASA系统分析精液常规参数,改良巴氏染色法分析精子形态;同时检测精浆生化(包括精浆弹性蛋白酶、精浆中性a-葡萄糖糖苷酶、精浆果糖)及精子DNA完整性。数据统计用Mann-Whitney检验方法。数据统计用Mann-Whitney U检验方法。结果:两组间精液量、精子总活力、PR、活动精子总数、正常精子形态比例、精浆果糖、精浆弹性蛋白酶组间有统计学差异(P<0.05),而其他精液参数均无统计学差异(P>0.05)。MG与精液量、精子总活力、PR、活动精子总数、正常形态精子比例精浆果糖呈显著负向影响相关(P<0.05),与精浆弹性蛋白酶及HDS呈显著正向影响相关(P<0.05)。结论:不育男性感染MG可降低精液量、精子活力(尤PR)、活动精子总数、正常精子形态比例及精浆果糖水平,同时更易引发精浆弹性蛋白酶及HDS的升高,但对精液其他参数均无明显影响。 Objective:The effects of mycoplasma genitalium(MG)on semen-related parameters of infertile man were studied retrospectively,providing theoretical support for the treatment of male infertility.Methods:Retrospectively analyze the male patients treated in the Affiliated Reproduction Hospital of Shandong University from September 2019 to September 2021.Real-time fluorescence simultaneous amplification and testing(SAT)detects MG RNA in a patient's urine,Computer-Aided Semen Analysis(CASA)system analyzes the general parameters of semen,papanicolaou Staining Method analyzes sperm morphology,automatic biochemical-immune analyzer detects seminal plasma biochemistry(including PMN-Elastase、seminal plasma neutral a-glucosidase、Seminal plasma fructose)and flow cytometry method detects the integrity of Sperm DNA.The data obtained were subjected to statistical analysis by MannWhitney-test.Results:There were significant differences in semen volume,total motility,PR,total motile sperm,proportion of normal sperm,seminal plasma fructose,PMN-Elastase among groups(P<0.05),but there were no significant differences in other semen parameters(P>0.05).MG was significantly negatively correlated with semen volume,total motility,PR,total motile sperm,proportion of normal sperm,seminal plasma fructose(P<0.05),MG was significantly positive correlated with PMN-Elastase and HDS(P<0.05).Conclusion:Infection with MG in infertile men can reduce the amount of semen,sperm motility(especially PR),total number of motile sperm,proportion of normal sperm and Seminal plasma fructose level,At the same time,it is more likely to induce the increase of PMNElastase and HDS,but has no clear effects on other semen parameters.
