慢性粒细胞白血病患者检出宽热振幅抗-IH输血策略及疗效评价

Strategies and efficacy evaluation of anti-IH transfusion with wide thermal amplitude detected in patients with chronic myelogenous leukemia

目的:探讨血浆中检出宽热振幅复合抗体抗-IH慢性粒细胞白血病患者输血策略及输血疗效。方法:应用血清学方法对1例意外抗体筛查阳性慢性粒细胞白血病患者进行抗体特异性鉴定,同时采用血清学方法为其选择适宜输注的血液并对输血疗效进行评价。结果:①该患者血浆中检出宽热振幅复合抗体抗-IH。②为该患者找到适宜输注的血液。③该患者输血后血红蛋白达到预期效果且无输血不良反应发生。结论:血浆中检出抗-IH患者应选择ABO同型、H抗原最弱且盐水法、抗人球蛋白法均相合的血液,能有效地降低或避免溶血性输血反应的发生,对临床安全有效输血有重要意义。

Objective:To investigate the transfusion strategy and efficacy in patients with chronic myelogenous leukemia(CML)with wide heat amplitude compound antibody against-IH.Methods:Serological method was used to identify the antibody specificity of a patient with chronic myeloid leukemia(CML)who was positive for accidental antibody screening,and to select the appropriate suspended red blood cells for transfusion and evaluate the efficacy of transfusion.Results:①Wide thermal amplitude anti-IH complex antibody was detected in the plasma of this patient.②The appropriate red blood cell suspension was found for infusion.③The hemoglobin achieved the desired effect and no adverse reactions occurred after transfusion.Conclusion:Patients with anti-IH antibody detected in plasma should choose blood with ABO homotype,the weakest H-Ag,combined with saline tube method and anti-globulin method,which can effectively reduce or avoid the occurrence of hemolytic transfusion reaction,and is of great significance for clinical safe and effective blood transfusion.
