青少年和成人上颌后牙区颊侧牙槽骨量的锥形束CT研究

The buccal bone quantity of adolescents and adults in the maxillary posterior region measured with cone-beam computed tomography

摘要 目的研究上颌后牙区颊侧的骨质情况,分析得出颧牙槽微种植体植入的适宜位点以及不同年龄组骨量分布的差异。方法本回顾性研究纳入111例曾拍摄锥形束CT的研究对象,测量第二前磨牙与第一恒磨牙之间、第一恒磨牙近远中颊根之间以及第一恒磨牙与第二恒磨牙之间的颊侧牙槽骨厚度和牙槽骨高度。结果第一、第二恒磨牙之间距离牙槽顶7mm处颊侧牙槽骨厚度最大,为(3.74±1.13)mm。成年人与青少年的颊侧牙槽骨厚度差异在第一恒磨牙近远中颊根之间距离牙槽顶7mm处[成人(2.78±1.27)mm,青少年(3.52±1.32)mm,P=0.004]、第一恒磨牙近远中颊根之间距离牙槽顶9mm处[成人(2.90±1.34)mm,青少年(3.82±1.78)mm,P=0.006]、第一恒磨牙与第二恒磨牙之间距离牙槽顶7mm处[成人(3.56±1.18)mm,青少年(4.05±0.98)mm,P=0.027差异有统计学意义,青少年大于成年人。不同年龄组牙槽骨高度的差异无统计学意义。结论①颧牙槽区微种植体的最佳植入位置位于第一恒磨牙与第二恒磨牙之间;②青少年上颌后牙区颊侧牙槽骨厚度大于成年人。 Objective To evaluate the buccal bone condition of the maxillary posterior region,hence to provide reference for optimal sites of infrazygomatic mini-implant insertion,and to assess the bone thickness differences among ages.MethodsFor this retrospective study,the buccal alveolar bone thickness and bone height were analyzed in 3 regions of 1ll subjects:between the second premolar and first molar,between the mesiodistal roots of the first molar,and betwen first and second molars.ResultsThe thickest buccal alveolar bone of(3.74±1.13)mm was observed between first and second molars at 7 mm from the alveolar crest.There are age differences in buccal bone thickness between the mesiodistal buccal roots of the first molar at 7mm from the crest[Adults(2.78±1.27)mm,Adolescents(3.52±1.32)mm,P=0.004],between the mesiodistal buccal roots of the first molar at 9mm from the crest[Adults(2.90±1.34)mm,Adolescents(3.82±1.78)mm.P=0.006],and between first and second molars at 7mm from the crest[Adults(3.56±1.18)mm,Adolescents(4.05±0.98)mm,P=0.027].It is significantly thicker in adolescents than in adults.However,there are no age difference in the alveolar bone height.Conclusions①Themost ideal placement site for infrazygomatic mini-implant is between the first and second molars.②The buccal bone thickness of adolescent is larger than that of adult.
