整体护理在牙龈癌患者中的应用效果

Application effect of holistic nursing in patients with gingival cancer

目的探讨整体护理在牙龈癌患者中的应用效果分析。方法选取2016年1月至2019年5月收治的牙龈癌患者104例,随机分为对照组和研究组,每组52例。对照组应用常规护理干预,研究组在此基础上应用整体护理干预,观察2组的满意率、口腔感染率、生存率、依从率、治疗有效率、并发症发生率情况,心理状态变化情况,炎性因子[白介素1β(IL-β)、肿瘤坏死因子-α(TNF-α)]水平和前列腺素E2(PGE2)水平变化情况、恢复情况以及生活质量评分情况。结果研究组满意率、生存率、依从率和治疗有效率均较对照组高,口腔感染率和并发症发生率均较对照组低(P<0.05);研究组干预后SDS、SAS评分,IL-1β、TNF-α、PGE 2水平均低于对照组(P<0.05);研究组住院时间、拔引流管时间、术后卧床时间均短于对照组(P<0.05);研究组各项生活质量评分均高于对照组(P<0.05)。结论牙龈癌患者应用整体护理效果较好,可以有效降低患者的并发症发生率,改善其不良情绪,提高其依从性、治疗效果和生存率,缩短其康复时间的同时降低龈沟液中的炎性因子水平和前列腺素E2水平,有利于各项生活质量评分的提高,值得推广和应用。

Objective To analyze the application effect of holistic nursing in patients with gingival carcinoma(GC).Methods GC patients(n=104)in our hospital from January 2016 to May 2019 were randomly enrroled.The study group(n=52)received conventional nursing+holistic nursing,but the control group(n=52)received the former only.The aiming was to compare satisfaction rate,oral infection rate,survival rate,compliance rate,curative effect,complication rate,mental state,inflammatory factor levels[Interleukin 1β(IL-1β),tumor necrosis factor-α(TNF-α)],prostaglandin E2(PGE2),recovery,and quality-of-life scores.Results The satisfaction rate,survival rate,compliance rate,and curative effect in the study group were markedly superior to those in the control group.The oral infection rate and complication rate in the study group were significantly lower as compared to the control group(P<0.05);the score of SDS and SAS scores and the level of IL-1β,TNF-α,and PGE2 in the study group were decidedly higher than those in the control group(P<0.05);the hospital stay,time to extubation and postoperative bedrest in the study group were obviously shorter compared with the control group(P<0.05);the quality-of-life scores in the study group were significantly higher than those of the control group(P<0.05).Conclusion The overall nursing has better effect for GC patients,which can effectively reduce complications,improve their bad mood,improve compliance,curative effect and survival rate.Shorten recovery and reduce inflammation in gingival crevicular fluid as well as PGE2 levels make contribution to the improvement of quality-of-life scores,worthy of promotion and application.
