慢性心力衰竭专科护理门诊的构建与效果评价

Construction and effect evaluation of chronic heart failure specialist nursing clinic

目的:探讨慢性心力衰竭专科护理门诊在慢性心力衰竭病人中的作用。方法:选取2019年1月—2019年6月本院出院的200例慢性心力衰竭病人作为对照组,实施常规门诊随访;2019年7月—2019年12月本院出院的200例慢性心力衰竭病人作为观察组,纳入本院心力衰竭规范化治疗系统,由心力衰竭专科护理门诊团队开展专科护理门诊服务。实施6个月后比较两组病人生活质量、自我管理能力、再住院率、满意度等。结果:观察组生活质量和自我护理能力明显高于对照组(P<0.001)。观察组3个月、6个月内再住院率低于对照组(P<0.05)。观察组病人满意度高于对照组(P<0.05)。结论:开展心力衰竭专科护理门诊满足了心力衰竭病人延续护理的需求,提高了生活质量和自我护理能力,降低了再住院率,提升了病人满意度。

Objective:To explore the effect of the specialist nursing clinic for chronic heart failure(CHF)in patients with CHF.Methods:A total of 200 CHF patients discharged from our hospital from January 2019 to June 2019 were selected as the control group and received routine outpatient follow-ups.Another 200 CHF patients discharged from our hospital from July 2019 to December 2019 were enrolledin the observation group and included in our hospital′s standardized treatment system for heart failure.Patients in the observation groupreceived the specialist nursing outpatient service for heart failure from the specialist nursing clinic team for heart failure.After 6 months of implementation,the quality of life(QOL),self-management ability,rehospitalization rate,and satisfaction of patientswere compared between the two groups.Results:The scores of QOL and disease self-management ability in the observation group were significantly higher than those in the control group(P<0.001).The rate of rehospitalization within 3 months and 6 months in the observation group was lower than that in the control group(P<0.05).Patient satisfaction in the observation group was higher than that in the control group(P<0.05).Conclusions:Specialist nursing outpatient services for heart failure mest the needs of heart failure patients for continuous nursing,improve the QOL and self-care ability,reduce the rehospitalization rate,and improve patients′satisfaction.
