重症感染性肺炎患儿血清维生素A、维生素D水平变化及其与疾病转归、肺功能关系研究

Study on the changes of serum vitamin A and vitamin D levels and their relationship with disease outcome and lung function in children with severe infectious pneumonia

摘要 目的探讨重症感染性肺炎(SIP)患儿血清维生素A(VA)和维生素D(VD)水平变化及其与疾病转归、肺功能的关系。方法选取自2017年5月至2021年6月收治的102例SIP患儿为重症组,同期选取102例轻症感染性肺炎患儿为轻症组。检测重症组和轻症组患儿血清VA和VD水平,比较两组患儿肺功能情况,根据SIP患儿病情恢复情况分为转归良好组和转归不良组,分析血清VA和VD水平与疾病转归的关系,采用Pearson分析血清VA和VD水平与肺功能的相关性。结果重症组患儿的VA和VD水平均低于轻症组,差异有统计学意义(P<0.05)。转归不良组患儿的VA和VD水平均低于转归良好组,差异有统计学意义(P<0.05)。重症组患儿的用力肺活量、第1秒钟用力呼气容积、最大通气量水平均低于轻症组,重症组患儿的肿瘤坏死因子-α、C-反应蛋白、降钙素原均高于轻症组,差异有统计学意义(P<0.05)。VA水平与肺功能(用力肺活量、第1秒钟用力呼气容积、最大通气量)指标呈正相关(r=0.687、r=0.722、r=0.557;P<0.05);VD水平与肺功能指标(用力肺活量、第1秒钟用力呼气容积、最大通气量)均归呈正相关(r=0.716、r=0.726、r=0.542,P<0.05)。血清VA和VD水平的升高能促进疾病转归。结论SIP患儿血清VA和维生素D水平降低,且与肺功能指标均呈正相关性,血清VA和VD水平的升高能促进疾病转归。 Objective To explore the changes of serum vitamin A(VA)and vitamin D(VD)in children with severe infectious pneumonia(SIP)and their relationship with disease outcomes and pulmonary function.Methods A total of 102 children with SIP admitted to the hospital were enrolled as severe group from May 2017 to June 2021,while 102 children with mild infectious pneumonia during the same period were enrolled as mild group.The levels of serum VA and VD in both groups were detected,and pulmonary function between the two groups was compared.According to the recovery of SIP children,they were divided into good outcome group and poor outcome group.The relationship between serum VA,VD levels and disease outcomes was analyzed.The correlation between serum VA,VD levels and pulmonary function was analyzed by Pearson analysis.Results The levels of VA and VD in severe group were lower than those in mild group,and the difference was statistically significant(P<0.05).The levels of VA and VD in the poor outcome group were lower than those in the good outcome group,and the difference was statistically significant(P<0.05).The levels of fast vital capacity,forced expiratory volume in 1 second and maximal voluntary ventilation in the severe group were lower than those in the mild group,and the levels of tumour necrosis factor-α,C-reaction protein and procalcitonin in the severe group were higher than those in the mild group,with statistical significance(P<0.05).VA level was positively correlated with pulmonary function indexes(fast vital capacity、forced expiratory volume in 1 second、maximal voluntary ventilation)(r=0.687、r=0.722、r=0.557;P<0.05),and vitamin D level was also positively correlated with them(r=0.716、r=0.726、r=0.542,P<0.05).The increase of serum VA and VD could promote disease outcomes.Conclusion The levels of serum VA and VD decrease in SIP children,which are positively correlated with pulmonary function indexes.The increase of serum VA and VD can promote disease outcomes.
