靶向药物治疗晚期非小细胞肺癌患者预后因素分析

摘要 目的 探讨分子靶向药物治疗晚期非小细胞肺癌(Non-small cell lung cancer, NSCLC)患者预后的影响因素。方法 回顾性分析89例经分子靶向药物治疗的晚期NSCLC患者的临床资料和随访结果,采用多因素回归分析法考察影响预后的因素。结果 89例晚期NSCLC患者,1年生存率93.3%(83/89)、3年生存率57.3%(51/89)。预后不良组性别、年龄、吸烟、远处转移、无化/放疗占比均高于预后良好组,差异有统计学意义;两组分期、基因突变及淋巴结转移差异无统计学意义,多因素分析结果显示:性别、吸烟、远处转移是靶向药物治疗晚期NSCLC患者预后不良的独立危险因素。结论 靶向药物治疗晚期NSCLC效果确切,但仍存在预后不良情况,主要受性别、吸烟、远处转移等因素影响,临床需高度重视。 OBJECTIVE To investigate influencing factors of prognosis in advanced non-small cell lung cancer(NSCLC)patients treated with molecular targeted drugs.METHODS We retrospectively analyzed the clinical data and follow-up results of 89 advanced NSCLC patients treated with molecular targeted drugs.Multivariate regression analysis was used to investigate the influencing factors of prognosis.RESULTS Among 89 patients with advanced NSCLC,the 1-year survival rate was 93.3%(83/89) and the 3-year survival rate was 57.3%(51/89).The proportion of gender, age, smoking, distant metastasis, and no chemotherapy/radiotherapy in the poor prognosis group was higher than that in the good prognosis group, and the difference was statistically significant.There was no difference in stage, gene mutation and lymph node metastasis between the two groups.Multivariate logistic regression analysis showed that gender, smoking, and distant metastasis were independent risk factors for poor prognosis in advanced NSCLC patients treated with targeted drugs.CONCLUSION Targeted drugs are effective in the treatment of advanced NSCLC,but there is still a poor prognosis, which is mainly affected by factors such as gender, smoking, and distant metastasis.
