摘要
目的:探究儿童钙化上皮瘤和常见皮肤肿物鉴别诊断中彩色高频超声的应用价值。方法:回顾性分析2021年4月~2022年4月本院收治的201例皮肤肿物患儿临床资料。所有患儿均实行皮肤肿物切除术治疗,术前接受彩色高频超声检查。统计彩色高频超声检查结果,并以病理学检查结果为金标准,评价彩色高频超声对儿童钙化上皮瘤的诊断效能,归纳彩色高频超声对不同类型皮肤肿物的诊断特征。结果:病理学检查结果显示201例研究对象中钙化上皮瘤占39.30%,非钙化上皮瘤占60.70%,非钙化上皮瘤中皮样囊肿1例、表皮样囊肿70例、脂肪瘤10例、纤维瘤11例,混合瘤1例,血管瘤29例。彩色高频超声对儿童钙化上皮瘤的阳性预测值、阴性预测值、灵敏度、准确度、特异度、约登指数分别为96.05%、95.20%、92.41%、95.52%、97.54%、0.897。结论:儿童钙化上皮瘤和常见皮肤肿物鉴别诊断中彩色高频超声具有较高应用价值,根据彩色高频超声检查结果可准确鉴别钙化上皮瘤与非钙化上皮瘤,确定儿童常见皮肤肿物类型,为临床治疗提供准确、可靠检验依据。
Objective:To explore the value of color high-frequency ultrasound in the differential diagnosis of calcified epithelioma and common skin tumors in children.Methods:The clinical data of 201 children with skin tumors treated in our hospital from April 2021 to April 2022 were analyzed retrospectively.All children were treated with skin tumor resection and underwent color high-frequency ultrasound examination before operation.The diagnostic efficacy of color highfrequency ultrasound in the diagnosis of calcified epithelioma in children was evaluated,and the diagnostic characteristics of color high-frequency ultrasound in different types of skin tumors were summarized.Results:Pathological examination showed that among 201 subjects,calcified epithelioma accounted for 39.30%,non calcified epithelioma accounted for 60.70%,and non calcified epithelioma included 1 dermoid cyst,70 epidermoid cysts,10 lipomas,11 fibromas,1 mixed tumor and 29 hemangiomas.The positive predictive value,negative predictive value,sensitivity,accuracy,specificity and yoden index of color highfrequency ultrasound for calcified epithelioma in children were 96.05%,95.20%,92.41%,95.52%,97.54%and 0.897 respectively.Conclusion:Color highfrequency ultrasound has high application value in the differential diagnosis of calcified epithelioma and common skin tumors in children.According to the results of color high-frequency ultrasound,it can accurately distinguish calcified epithelioma from non calcified epithelioma,determine the types of common skin tumors in children,and provide accurate and reliable test basis for clinical treatment.
