摘要
目的 探讨神经电生理联合高频超声在桡神经损伤诊断中的应用价值。方法 选取2019年8月10日至2021年8月10日我院收治的32例疑似桡神经损伤患者作为研究对象。所有患者均给予神经电生理、高频超声诊断及手术治疗。分析手术病理、神经电生理、高频超声及神经电生理+高频超声的诊断结果;以手术病理结果为金标准,比较神经电生理、高频超声及神经电生理+高频超声对桡神经损伤的诊断效能。结果 32例疑似桡神经损伤患者中,手术病理诊断出桡神经损伤25例,非桡神经损伤7例;神经电生理诊断出桡神经损伤23例,非桡神经损伤9例;高频超声诊断出桡神经损伤21例,非桡神经损伤11例;神经电生理+高频超声诊断出桡神经损伤24例,非桡神经损伤8例。神经电生理+高频超声诊断桡神经损伤的灵敏度、特异度、准确度、阴性预测值、约登指数高于神经电生理和高频超声诊断(P<0.05)。结论 神经电生理联合高频超声对桡神经损伤的诊断效能较高,值得推广和应用。
Objective To explore the application value of nerve electrophysiology combined with high frequency ultrasound in the diagnosis of radial nerve injury. Methods A total of 32 patients with suspected radial nerve injury admitted in our hospital from August 10, 2019 to August 10, 2021 were selected as the research objects. All patients were given nerve electrophysiology, high frequency ultrasound diagnosis and surgical treatment. The diagnosis results of surgical pathology, nerve electrophysiology, high frequency ultrasound and nerve electrophysiology + high frequency ultrasound were analyzed;the diagnostic efficacy of nerve electrophysiology, high frequency ultrasound and nerve electrophysiology +high frequency ultrasound in the diagnosis of radial nerve injury were compared with the surgical pathological results as the gold standard. Results Among the 32 patients with suspected radial nerve injury, 25 cases of radial nerve injury and 7cases of non-radial nerve injury were diagnosed by surgical pathology;23 cases of radial nerve injury and 9 cases of nonradial nerve injury were diagnosed by nerve electrophysiology;21 cases of radial nerve injury and 11 cases of non-radial nerve injury were diagnosed by high frequency ultrasound;24 cases of radial nerve injury and 8 cases of non-radial nerve injury were diagnosed by nerve electrophysiology + high frequency ultrasound. The sensitivity, specificity, accuracy,negative predictive value and Youden index of nerve electrophysiology + high frequency ultrasound in the diagnosis of radial nerve injury were higher than those of nerve electrophysiology and high frequency ultrasound(P<0.05). Conclusion Nerve electrophysiology combined with high frequency ultrasound has a high diagnostic efficiency for radial nerve injury,which is worth of promotion and application.
